All right, so let's dive into an OG topic here at the channel. We're going to get into methylene blue today, specifically methylene blue stacks. So, a stack is when we combine one molecule with another or compounds. In this case, we're going to talk about things that you can combine with methylene blue, how it may work, how it may not work, things not to do, etc. So, I want to frame these all and these are questions that we get quite regularly all the time. We get them all the time.
And so, we're getting questions about, "Can I do methylene blue at X?" "Can I do, can I give methylene blue to my dog?" All of that kind of stuff.
Let's start first with a combination that is semi-featured in BlueCotine. Methylene blue in the context of using it with caffeine. Methylene blue is fantastically supportive for the mitochondria, which is the part of the cell that makes energy. It helps with energy production, even if energy production is not going so well in the mitochondria, as well as detoxification, so helping you kind of combat the stress that happens when you do make energy. Nothing but works as an energy enhancer and an antioxidant at the same time. So, combining it with something like caffeine is very interesting here because caffeine, what it's doing, caffeine is stimulating the body to make more energy. It's making you more wakeful, increasing the energy because of neurotransmitters that are released as you're becoming more wakeful and depressing other neurotransmitters like adenosine that would make you feel sleepier. So, the combination is actually fantastic, synergistic, right? You have the caffeine that's giving you the stimulation, the wakefulness, and then you also have the methylene blue, which is giving you part of that, but it's also supporting you on the other side so you don't get a crash. Many people that know, I know that you know this, Boomer, like when you're drinking a lot of caffeine, you can get a crash, even if you're just having pure caffeine, but especially if you're having like energy drinks and sugar with it and all those other kinds of things. Methylene blue, caffeine, again, very fascinating combination.
One of the compounds that has come back and has been around forever but has had a bit of its heyday this year, aside from methylene blue, is creatine monohydrate.
So, creatine monohydrate is something that bodybuilders used for decades, right? I remember taking this when I was probably 14, 16 years old, uh, and still take it. And now we're kind of looking at it from both a physical performance standpoint, but also a brain performance standpoint. There's also some studies now to suggest that it'll help in fatigue states and making sure that the brain, in particular, uh, doesn't go into a detrimental state. So, creatine, methylene blue, good combination.
Yeah, it's a fantastic one, right? Because creatine is, it's the unsung hero of our capacity inherently to make more energy when we need to. And so that's why Boomer was just mentioning a study that was just recently done. I think the dose is around 20 grams of creatine in the morning actually could mitigate the effects of sleep deprivation. And that's because you gave your brain another energy source, a capacity that it didn't have before. And so, how do you combine this with methylene blue? Well, not surprisingly, methylene blue is going to increase your energy efficiency as well. It's going to increase your detoxification. And so that's going to support mitochondrial function while creatine gives you another source of making that energy as well. So, there's a reserve there that the creatine is giving you. And as a result of this, you get this sort of holistic way of supporting brain function. So, in a lot of my patients that are fatigued in the morning, have a hard time sleeping, that are stressed, creatine in the morning is usually a must these days, along with methylene blue as well to support mitochondrial function. So, they're fantastically synergistic. You can take them at the same time, you can take them at different times. It doesn't really matter. What I typically have people do starting off with is to take the creatine in the morning and also the methylene blue in the morning. So, low doses of methylene blue, four, eight, 16 milligrams. You've got to be careful on your dose. Once you start getting above like 30, 40, 50 milligrams of methylene blue, then it actually can cause more stress on the brain than support it. This might seem like surprising to you because most people that are talking about methylene blue are talking about these doses as being therapeutic, 40, 60, 70 milligrams. But what we found doing this, so we were the, the first company that came out with a commercial methylene blue product back in 2020. We have a lot of clinical experience. We have tons of practitioners that we work with, you know, thousands that have been using this in clinical practice. Once you get to that point where you're about a milligram per kilogram, so about 50 to 70 milligrams of methylene blue, you get anti-infective capacity there, but you're also causing more stress on the system because you're making more hydrogen peroxide.
There's also something that you drew attention to pretty early on on those higher doses of methylene blue with biofilm.
Yeah. So, the biofilms are these films that are made in various areas in our body. We have biofilms in our teeth. So, you wake up in the morning, you get that filmy feel on your teeth. That's biofilms. They have biofilms in your gut as well. And these are normal. We have these are biofilms where a place where hopefully good bacteria reside, mucus resides, other cellular processes and other cellular pieces are also there as well. But if these things become pathogenic, they actually keep certain types of things outside of the purvey of your immune system. So, they hide things. And so, sometimes we want to break up biofilms. And these higher doses of methylene blue are biofilm busters. So, they can bust biofilms in the gut, they can bust a biofilm in the bladder as well. Biofilms in the bladder can happen as well as the gut. But you have to be mindful, right? Because you don't always want to do that. Like, we get a lot of questions about, "Well, is methylene blue going to disrupt my gut microbiota and kill things off?" And I say, look at these low doses, four to like 25 milligrams or so, very safe. We haven't seen any detrimental effect. In fact, we've seen positive effects overall, probably because of mitochondrial efficiency. But these higher doses over 30 to 40, but definitely a milligram per kilogram of 50 to 70 milligrams of methylene blue are going to cause biofilm disruption over time. They are going to cause a decrease in your microbiotic colonies as well. So, this may be a benefit depending on what's going on. But in essence, from a cognitive enhancement perspective, these lower doses, four, eight, 16 milligrams are truly key.
Another trending topic, and this goes on to question, I guess, three at this point here. Peptides. It's been a trending topic in the health world for the past several years. And the way I like to think of peptides is a string of amino acids that helps complete a recipe for the body to make something. This could be growth hormone, as an example. Um, there are several others. BPC-157. The combinations of peptides with methylene blue. Thumbs up, thumbs down.
I mean, total thumbs up. I mean, of course, with peptides, you have to be careful where you're sourcing them from. Peptides are harder to get now because of FDA regulations related to certain peptides. I mean, good story for me. I partially tore my calf about three weeks ago. Uh, that was fun. Good times. Uh, limping around in a lot of pain. But what I was doing, uh, for a long period of time, well, maybe a couple, just a couple weeks was higher-end dose of methylene blue along with some peptides to help with healing, BPC-157, TB500. And talking to the docs of people that I knew, they thought I'd be maybe about a month before I was walking without a limp. And I'm two weeks out and I'm doing well. I can't run, that's for sure. But I can walk without looking like I'm...

So, the methylene blue story with peptides is, of course, going to be nuanced. In my experience with that particular incident, I was using higher doses of methylene blue up to about 30 milligrams, 15 to 30 milligrams a day for about two weeks. And I was also using BPC-157, TB500. And so, the thing about methylene blue at the slightly higher doses that I was taking for an acute issue, the anti-inflammatory capacity is quite high. And so that's why I took it there. But in essence, if you're looking more for mitochondrial support, you can combine it with other peptides that are working on mitochondrial function and see a significant benefit. Now, the key, and this is the challenge with a lot of people in our world, is like, I have one guy I spoke to a couple days ago. He was on seven different mitochondrial peptides and he was taking methylene blue. I'm like, "Is all of this necessary?" You know, something.
Yeah. How do you know which one's working? There's a little bit of an isolation story there, too.
Yeah. I mean, there's, there's so many different things that can affect the mitochondria. There's different peptides that can help with inflammation. There's other peptides that can help with specifically mitochondrial function. There's others that can help with the stress response. And so, a lot of this is nuanced. But in essence, the cool thing about methylene blue, I think, is that it can add, be added to almost any regimen that you're on to support mitochondrial function, which means that low doses support your ability to energy produce and detox. So, I tell my patients, energy, detox, energy resilience, and methylene blue. So, you can add it to your protocols very early on and start slowly titrating that. And the other key is titration. So, starting off at low doses like four milligrams, increasing it every three to five days until you find the right dose for you. And the other piece of this that's important is that many things out there need to be titrated and you might need to stay on it for like a month or something like that to know whether it's working. When you find the right dose of methylene blue, you should know it. It shouldn't be one of the things where like, "I don't know, maybe I feel a little bit better." Like, no, especially for like a chronic kind of thing where you have mitochondrial dysfunction. When you find the right dose, it's pretty obvious, and that's the dose that you need. And some people need that dose daily for a period of time until they can actually optimize themselves in other ways. Other people that are more optimized can just take it when they have more stress, like travel or jet lag or coming to altitude or or other areas of stress or poor sleep and things like that.
One that probably doesn't immediately come to everybody's mind is the idea of using methylene blue with a GABA agonist, if you will, something to relax you. Is there anything there?
Yeah, I think the idea really is that we need to optimize mitochondrial function, but we can't do it well. We don't do it well if we don't optimize the capacity for you to get into what's called parasympathetic mode to rest, digest, detox. That's our nervous system that allows us to do that. Most of us are in sympathetic overload at all times, which means that we're in fight or flight. We deplete neurotransmitters, we deplete hormones, and we screw up the mitochondria even more. And so, if you don't address that sympathetic overdrive, you're not going to see the benefits even from methylene blue over long periods of time. You'll see a benefit, but you won't see the benefit you anticipate. And this has been my clinical experience. So, the GABA system downregulates that stress response. It downregulates your ability to, improves your ability really to balance that nervous system so that you have more sympathetic downregulation, you have more parasympathetic upregulation. And as a result of that, you have more capacity to feel relaxed and also downregulate the nervous system so you can detox, you can digest, and you can recover.
So, thus far, we've talked about methylene blue and peptides, we've talked about methylene blue and GABA, we've talked about methylene blue with creatine, as well as methylene blue and caffeine. I, I want to just get a little bit into our Department of Defense, right? And so, the Department of Defense has the largest budget in most countries unless you're in Bhutan. But if you're talking about the body's Department of Defense, we're talking about the immune system and immune system function. What about using methylene blue in combination with something like a cordycepin?
This is a big one that I use in clinical practice. The thing about our immune system, as you said, it takes up a huge amount of energy. If you don't have the support that you need, those immune cells aren't going to work very well. And so, methylene blue is something I'll give during the day, typically in the morning. Sometimes I'll dose it twice a day. So, I'll take it in the morning and then maybe at 12 o'clock, one o'clock in the afternoon as a way to give energy support and anti-inflammatory support throughout the day and give people more energy. Cordycepin, which is the active ingredient, the most potent ingredient in the Cordyceps mushroom. Cordyceps, as a mushroom, has been used in Chinese medicine for thousands of years actually as an anti-inflammatory, as an immune system enhancer, as an energy enhancer as well. But cordycepin itself is much more powerful than the m than the mushroom as an anti-inflammatory, as an immune system modulator, as an anti-infective, and it works directly in the mitochondria as an antioxidant as well, kind of like an adaptogen in various ways. So, what I'll typically do, um, is I'll give the methylene blue during the day and then the cordycepin at night because not only does it do all the things I just mentioned, it also increases deep sleep because it works just like adenosine in the body. Adenosine has a number of different functions, but as a neurotransmitter, adenosine helps you feel tired. It gives you sleep pressure, makes you want to fall asleep. And so, taking the cordycepin at night increases your deep sleep. And of course, sleep itself is anti-inflammatory and detoxifying. But you also have the immune system modulating aspects of the cordycepin. So, you're decreasing inflammation. You're downregulating pathways like TNF alpha and IL-6 that are coming from the NF-kappa B pathway. You're downregulating things like mTOR as well as and upregulating one another one called AMPK. It's another process, another pathway that increases our capacity to decrease inflammation. So, it's a fantastic way to support inflammatory cascades at night. People wake up feeling so much better, you know, using that at night along with a combination of methylene blue.
Longevity circles. This three-letter word has been tossed around for many moons now. People were doing IVs of it for a long time. That seems to have gone by the wayside somewhat. Uh, and now there's sort of different forms that are being introduced. Methylene blue and NAD. Why or why not?
So, the answer is yes, in the sense that what methylene blue can do is it can help recycle NAD. So, the problem as we get older is that the NAD pool, the amount of NAD that we have, goes down. So, that means we have less capacity to take electrons from our food. That's why we eat fats and carbohydrates, specifically, is to take the electrons off the food, bring them into the electron transport chain of the mitochondria. NAD is one of those carriers. So, it's got two different forms. It's got an oxidized form, NAD+, and a reduced form called NADH. And so, it actually picks up the electrons from your food and then brings those electrons over to your electron transport chain to help you make energy. The problem is, if you don't have enough NAD around, you can't do that very effectively. So, the, the idea is that if you can give more of an NAD source, you're going to give more capacity to make energy. And what methylene blue can do there is help recycle that NAD more efficiently so you can bring it back to the citric acid cycle in the mitochondria. So, you've helped make more energy. So, it has that capacity. And even still, what's actually really important here is that if you have an issue with your complex one, which is the first complex on your mitochondria, this is the one that takes most of your electrons from your food. This is the one that NAD donates its electron to. If complex one isn't working very well, a good example is taking metformin, a a diabetic drug. What's going to happen is that that NAD is not going to be able to recycle itself because there's no place to send its electrons. And so, what can happen is that it gets a lot of stress on the system and you can't make a lot of energy effectively. So, what methylene blue can do in that case is actually recycle that NAD to get it back to the NAD+ even if that complex one isn't working very well and actually can donate electrons further down the chain to complex 3 and 4 to kind of bypass that. And complex one dysfunction, not just because of metformin, infections, toxins, stress in our environment. It's one of our most sensitive complexes to environmental stimuli. And if you have insulin resistance as well, and you're also going to cause complex one destruction. So, taking NAD as a supplement is controversial. There's different forms of it. There's NAD+, there's NMN, there's NR, there's IV. And there's some worries, especially now, that taking too much NAD might actually be a detrimental aspect of doing something because NAD is also not only running normal cells, but also can actually run cancerous cells or precancerous cells as well. So, there's some more interest in the nuanced discussion about this. But in essence, I think that's the way methylene blue can play. I think it's here a good point to interject that at Health Optimization Medicine and Practice, our sister nonprofit, what we do is we teach people essentially, "Test, don't guess." Right? And you can do a metabolomics test and really measure whether or not you need NAD in the body. And you can see it through a series of vitamin B metabolites, right? That would allow you to kind of hone in on whether or not you need to invest in yet another supplement, right?
Yeah, I mean, and the good thing you just mentioned, like vitamin B3, that niacin is your precursor for NAD. So, ideally, you give the precursor because then it can be converted into the NAD if you're supporting the system. That's likely the best way to do it. And we think about this as, as we formulate. We have something called Vitamin B3 GABA, so, or nicotinol GABA, which is a vitamin B3 attached to the GABA, and that gets across the blood-brain barrier as opposed to GABA supplements. The B3 is mildly activating because of that NAD, and then you have the GABA that's relaxing. So, we think about this in in various contexts as a result of wanting to make sure we're supporting the whole system.
There's one combination that I think is sort of a no-go, and that's methylene blue and SSRIs.
Yeah.
Uh, can we talk a little bit about that?
So, methylene blue is a monoamine oxidase inhibitor, which means that it increases serotonin, norepinephrine, and dopamine. And so, SSRIs, anti-depressants, SNRIs are serotonin-norepinephrine reuptake inhibitors. These can potentially interact with methylene blue because they're working on similar neurotransmitters and increasing them together. Now, very low doses of methylene blue are likely quite safe, but if you're on an SSRI, SNRI, or dopaminergic drug like a Parkinson's medication, you really need to be working very closely with a provider if you're going to start any methylene blue. In essence, the, the risk that is discussed in the literature is something called serotonin syndrome, where somebody gets into like too much serotonin, causing elevated blood pressures, heart rates, and it could be potentially deadly. But the thing about serotonin syndrome is that it's extremely rare. If you ask a hundred doctors if they've ever seen it, they've been in clinical practice for 20 years plus, maybe two or three people will actually raise their hands in the audience, and maybe one of those people actually saw serotonin syndrome because it's very, very rare. So, the most common side effect you'll find is people have GI distress because most of the serotonin we make is not actually in the brain, it's actually in the gut. But in essence, the only studies that have shown serotonin syndrome with methylene blue have been using IV methylene blue. IV methylene blue is going to get to the brain much faster than obviously taking it orally or swallowing it, whatever you'd like to do. So, short story is, if you're using an SSRI, SNRI, you can work with a practitioner very closely as long as you're using very low doses of methylene blue. But do not do this on your own. On the other side of this, I would say that we have integrative psychiatrists that we're working with that are weaning their patients off these kinds of medications with very close supervision and using methylene blue in its place because of the mood-boosting capacity that methylene blue does. And there's been studies in depression and bipolar specifically showing how methylene blue can work. And the final piece there is it's not only the neurochemistry, okay, it's also the mitochondrial support that methylene blue is giving at the same time.
Two final questions to wrap this up. We're going to talk about cycling and quality. Uh, cycling. If I'm somebody who takes methylene blue, should I be taking it daily or should I look to kind of cycle it off?
What I have to think about here is that I think that some people need methylene blue some of the time if you're more optimized, and other people need methylene blue some of the time for a longer period of time. And the reason for that is that it depends on where you're going and where you're coming from, right? If you're coming from a pretty optimized place, then you may only need methylene blue periodically when you have more stress. You have, you're going on an airplane, you have more stress at home, you're not sleeping as well. Using methylene blue in those cases is going to mitigate the mitochondrial stress, especially from flying. Like airplane travel is such a stressful thing. You're low oxygen on a plane, you have bad air quality, you're changing your circadian rhythms, your gut's not happy.
Kind of view it during travel as like my insurance policy, right? How do I protect my cells from like the 300 other people in this plane that statistically are probably not nearly as healthy as I am?
Yeah. So, you have the capacity to use methylene blue in this kind of these short-term ways to help with travel, stress, etc., if you're more optimized. But if you're coming from a place where your mitochondria need significant support, you may need to take methylene blue more of the time in the beginning and hopefully be able to wean it off over time as you're optimizing your diet, your lifestyle, your nutrition, etc. And the cool thing about methylene blue, these lower doses, is that it gives you enough support that you start feeling better. All of a sudden, you feel like you can take a walk around the block. All of a sudden, you can feel like you can change your diet just a little bit or you can maybe...
Recall memories, right?
Maybe have something a little bit better. Yeah. So, like all of a sudden your memory's better, like cognition's better. So, you feel better and then you're able to peel it off over time. But when it comes to cycling, the higher the dose that you're taking, the more you have to be thinking about cycling. What I, my rule of thumb these days is that if you're taking more than about 25 or 30 milligrams of methylene blue on a daily basis, you should likely cycle it off at least once a week. If you're taking higher than that, about higher than 70 milligrams a day, then definitely cycling it off at least twice a week is likely best. But if you're taking more than 30 milligrams a day, I really think you should be following with a practitioner pretty closely to kind of understand why you're taking it. Now, some people really do benefit from these doses at high doses for long periods of time. I've seen many benefits at these high doses for long periods, but the higher the dose you're taking, the more cycling you need to do. At these lower doses, four, eight, 16 milligrams, you don't really need to cycle. Although we'll find a lot of people will do is maybe take one or two days off per week and not take it. But there's really no clinical experience that we say that absolutely that's required.
Last part is about quality. And you know, we've been at this. We released BlueCotine in February 2020, just blue shortly after that. Uh, we were the first to really commercialize methylene blue.
And since then now we see various influencers releasing their own methylene blue. We see Amazon products coming out on methylene blue. We see, uh, people getting into the space because it is trending.
Right.
What are some of the concerns that we need to be wary of? And, you know, after you go through that, I'll go through some of the tests we've done on these various products.
Yeah. Yeah. Yeah, I think from like a high-level perspective, methylene blue is a synthetic molecule, which means it doesn't come from the earth directly. It was made in the lab and it was the first drug registered with our FDA back in 1897. And so, some people don't like synthetic things. I get that. But my response to this is that not everything that's synthetic is bad for us, just like not everything natural is good for us. We don't live in a natural world. We live under bad lighting. We have bad food, toxins in our environment, thousands of chemicals that are being put in our water, all these kinds of things, all on a daily basis, right? So, not everything is natural anymore. So, as a result of that, I think we need sometimes synthetic things to help us. And as long as you keep it very low dose, methylene blue is extremely safe, extremely well tolerated. The only thing you'll find is that you'll get some blue pee, and that's the only thing you'll have to, you know, be somewhat surprised about if you didn't know ahead of time, right? But because it's synthetic, during the manufacturing process, it could be contaminated with heavy metals, especially mercury, cadmium, arsenic, and lead. Okay? And so, this is why you do not want to drink fish tank cleaner. When we first looked at methylene blue back in 2019, we had a really hard time finding a pharmaceutical grade company that would make methylene blue to our specs. And so, fish tank cleaner, industrial grade, chemical grade, you do not want to have those kinds of methylene blue because all of them are going to be contaminated with at least 10% or higher of heavy metals. So, do not take those products. In the beginning of the pandemic, we had a great headline. I remember, remember this one. And said, um, "Biohackers drinking fish tank cleaner to reverse their age." Great, great, great clickbait.
That was actually the New York Post. And funny enough, this year, the New York Post actually came out with a more positive article on methylene blue, too.
Yeah. And this kind of goes into how we understand the quality and the testing. Maybe you can go into some of that, Boomer.
Yeah, sure. So, when all of these products came onto the market, of course, what we were concerned about was product quality. We know the process that methylene blue is made. We know the questions to ask, what to look for. And so, when we went and purchased sort of the top 10ish methylene blue products, what we were testing for was, uh, what's called high-performance liquid chromatography, or purity. We were also looking at microbial analysis. So, the presence of different microbes in in the product, as well as heavy metals. And of these 10, none of them actually, uh, tested to spec. And what does that mean? They usually didn't meet label claims in terms of dosing. They were either, in one case, way too high, which has a detrimental effect if you don't know what you're taking, or way too low. And all of them had some sort of issue with the heavy metals that Scott just mentioned. Some of them had higher arsenic, some of them had higher lead, some of them had higher mercury. And there, I guess, is a way, and something that I would like to encourage anybody watching this, demand more of your supplement companies. When you buy a product, you're investing in yourself. And you're buying a product because you want it to make you better. And so, when you're approaching a supplement company, you can ask for these tests. Do you have high-performance liquid chromatography instead of NMR? Uh, do you have heavy metal testing? Do you have that microbial analysis? And they should provide it to you. If they don't, well, go buy another supplement because there's plenty of companies out there now doing this. And everybody should hold these companies to a higher standard. And that's regardless if it's methylene blue or vitamin D. You should go out there and ask more of your supplement companies.
Yeah. I mean, what, what Boomer is talking about here is is called the Certificate of Analysis, or COA. And every company, every supplement company needs to have a COA. Actually, you don't really need to, do they? Actually, there's really no regulations, but they should have a COA that says exactly what Boomer said. This is the potency, this is the purity, this is the microbial analysis, and be able to give that to you. The problem is, many of the raw ingredients coming into the United States are coming from other countries. This is not inherently a bad thing, but what it really means is that China, Japan, Korea, India, South Africa, we have all different suppliers all over the world. For our methylene blue, it mostly comes from Japan and Korea, for example. But we've had a manufacturer that we've been working with for the last five years that we trust inherently, but we trust and verify, which means that we don't trust the Certificate of Analysis that comes from their lab because we don't know the company that was in Japan or in Korea that did it. Is it real? Is it not real? And there's been tons of recent exposes, especially one I saw recently on creatine gummies, right? Where creatine gummies coming to the United States have zero amount of creatine, even though the Certificate of Analysis from the company that was providing the ingredient said it was exactly what it said it was. So, you just don't know until you know. And so, what we do is we trust because we have a trusted source, but we verify. So, every single batch of methylene blue that we get, we don't trust the, the COA that we get from the manufacturer. We do another independent test here in the United States to confirm purity, potency, and micro, every single time with all of our ingredients, not just methylene blue, but everything that goes into our products. And we publish all this stuff on our website. It's all freely available. And it should be for every single supplement company that you're working with because you can forge those COAs. You cannot really know, even as a company, like you might think that a COA is great. I mean, I remember in the beginning, I remember in the beginning we tried to use a couple different methylene blues we first got, and they were not at all what they said they were.
Yeah, it, it's something that we're quite proud of here at prescriptions is the quality, the precision dose, physician formulated, pharmaceutical grade.
