This lesson is on what to avoid if you're taking low sartin, which is also known as Kosar. Before we talk about what to avoid when taking this medication, let's discuss what lowartin is, how it works, and some of the more common side effects of its use. So, loartin or Kosar is an angotensin receptor blocker, or an ARB medication. It's used to treat hypertension, or high blood pressure, and more specifically has a very beneficial effect for helping with diabetic nephropathy, which is kidney disease from diabetes.
Now, the mechanism of action of lucartin is the following: It inhibits action at andotensin 2 receptor. This is the reason why it's called an andotensin receptor blocker. And when it inhibits action at this receptor, it prevents andotensin 2 mediated effects. So, angotensin 2 has many different effects in the body. One is that it will cause vasoc constriction, or this is where the blood vessels or the arteries will tighten. They'll squeeze together. So, if we're inhibiting that process, we're going to get what we call vasoddilation. So, this is where the blood vessels open up. The diameter of the blood vessel increases. So, we're going to get a reduced pressure inside the blood vessel. It also has effects on the release of another hormone called eldoststerone, which can have other important effects. And it can also affect areas in the brain, including the hypothalamus, to increase fluid intake. So, if we're preventing all these processes, this is how low certain can have its effects on lowering blood pressure.
However, because it acts through these mechanisms, it can cause mild and/or severe side effects. These include the following: fatigue, hypotension, or a low blood pressure. So, especially if you're taking too high of a dose of low certain, you can get your blood pressure corrected too much. So, this can cause weakness, dizziness. We can also see chest pain as a potential side effect, and in some cases, hypoglycemia, or a low blood sugar level. And we can also see hyperc calmia, or high potassium level in the blood. So, all of these will be related to some of the effects we're going to see with regards to what we want to avoid throughout this lesson.
Before we talk about what to avoid, though, we need to talk briefly about the metabolism of losartin. So, loartin is going to be metabolized by an enzyme in the liver known as CYP2C9. And when this enzyme metabolizes loartin, it's going to metabolize it into a metabolite known as E3174. This is actually the active metabolite of loartin. This is where loertin actually gets most of its effects, and some of loertin will be excreted renally, which means that it'll be excreted through the kidneys. About 4% will be excreted renally, and then loertin also has a halflife of 1.5 to 2 hours.
So, let's talk about what to avoid when taking loartin. So, one of the things you want to avoid when taking loartin is ibuprofen. So, ibuprofen is a non-steroidal anti-inflammatory drug, or NSAID. And the reason you want to avoid ibuprofen is because losartin and ibuprofen both increase serum potassium levels. We talked about the fact that low cartin can increase serum potassium levels, causing hyperc calmia. Well, taking it along with ibuprofen, especially a lot of ibuprofen for a long period of time, we can have higher and higher levels of potassium in the blood. So, that can increase our risk for cardiac arhythmias. And low ibuprofen itself can also reduce low certain mediated effects as well. So, you want to be cautious when taking ibuprofen along with low certain.
Another medication you want to look out for when taking low certain is aspirin. So, aspirin is also an NSAID medication. And aspirin also can increase serum potassium levels. So, it has that additive effect with lowartin. Low certain itself can increase serum potassium levels. Aspirin can also increase serum potassium levels. And especially if you're taking a lot of aspirin, you want to keep an eye on your potassium levels. So, you again end up having a higher risk for having higher serum potassium levels, or hyperc calmia, which can increase your risk for cardiac arhythmias. And it can also have other similar effects, like ibuprofen. So, it can have effects on the way lowartin works as well. And other nades can also have similar effects. Some of these include dicloanac and celoxib. This also goes along with the fact that you want to avoid taking potassium supplementation when using low certain as well.
So, a lot of times, patients might be on potassium supplementation in the past, but they have low certain added to their medication regimen. So, you want to look out for this potential interaction again because low certain itself can increase serum potassium. And then that extra supplementation can lead to, or increase the risk for, hypercalemia.
Marijuana or cannabis use can also have important effects on how Oartin works. So, more specifically, canabidadial in cannabis can affect metabolism of low sartin. And the reason is because canabidial can inhibit activity of CYP2C9, that important enzyme we talked about before, and metabolizing low sartin to its active metabolite E3174. So, because we're inhibiting that potential enzyme, we're inhibiting the metabolism to that active metabolite, we're going to reduce low certain mediated effects. So, we're not going to get some of the blood pressure effects from low start like we should, and we will also have reduced effect of that protection on the kidneys with regards to diabetic neuropathy, for instance.
Other medications you want to avoid if you're taking low sartin include ACE inhibitors. So, ACE inhibitors are also anti-hypertensive medications. If you're taking anotensin receptor blocker, you're not going to be taking ACE inhibitors. But if you do happen to be on these, you want to inform your medical care provider because these can interact. So, some of the medications in the ACE inhibitor category include the prill medications: capipril, analopril, fosinipril, lysinopril, and pindipril. And the reason you want to avoid them is because they both affect the renin andotensin system. So, they cause increased effects of each other. So, this can lead to excessive effects on reducing your blood pressure, for instance, and this can cause hypotension, low blood pressure, and patients can become very dizzy and can fall. So, you want to look out for if you are on the nace inhibitor and you start low certain; you want to look out for this interaction again due to hypotension, but also due to hyperc calmia, or high potassium levels in the blood, because asinirus can also increase potassium as well.
Another medication you want to avoid if you're taking lucartin is carbomazipene. So, carbomazipene is an anti-seizure medication. It's going to be used for particular conditions, like trigeminal neuralgia, and it's going to reduce low certain function by reducing CYP2C9 activity. Again, it's reducing that enzyatic activity, preventing low certain from being metabolized into its active metabolite. So, you're going to get reduced loertin mediated effects.
Other medications you want to look out for when taking Lucartin include lithium. So, this is a medication used to treat bipolar disorder. And the reason you want to look out for it is because loartin can increase toxicity of lithium. Lithium is going to be renally excreted. It is excreted from the kidneys. So, loartin can have effects on the kidneys where it can lead to reduction in excretion of lithium. And lithium is very, very important to keep an eye on. It has to be taken at proper doses, and you definitely want to look out for this potential interaction.
Another medication to look out for if you're taking low certain is phenito. Phenitoin is also another anti-seizure medication. Phenito has a couple different effects. It enhances CYP2C9 enzyme, but it also enhances CYP3A4 enzyme, another liver enzyme. So, because it enhances CIP2C9, you might be thinking that it actually increases low certain effects because low certain is going to be metabolized into that E3174 active metabolite. But because it has effects on other enzymes in the liver, it can actually end up leading to reductions in E3174 levels as well. So, it reduces low certain; it reduces E3174 levels, and thus reducing low certain mediated effects. So, you're not going to get that blood pressure medication control like you're wanting.
Other medications to look out for with Oartin include amiotarone. This is a medication used to treat cardiac arhythmias. This particular medication inhibits CIP2C9 metabolism. So, it's going to be lead to a reduction in low certain mediated effects, again due to reduced levels of E3174.
Another medication that can cause effects with lowartin is the medication loexedine. Lexidine is an alpha 2 adinuric receptor agonist. This is a medication often used to treat opioid withdrawal, and it has its own effects. It's not going to act through effects on liver enzymes, but it's going to cause increases in effects of low certain mostly through additive effects. So, lexidine has hypotensive effects itself. So, if you're taking loexedine plus low certain, you can get excessive hypotensive effects. So, you can get very low blood pressures, and you can also get low heart rates as well.
Beta blockers can also interact with low certain use as well. Beta blockers include the following: dissoperol and carvdalol. These are used to treat blood pressure and heart rate. And these medications can lead to increased serum potassium levels themselves. But again, we know that if you're taking loartin, low certain can also increase serum potassium as well. So, if you're combining beta blockers and low certain, you can get higher serum potassium levels. So, you want to keep an eye on your serum potassium if you're on beta blockers and low certain.
And finally, antifungals can also interact with lartin as well. These include the aol medications: itchonazol, ketoconazol, or the connonol medications. These antifungals inhibit cip2c9 enzyatic activity. We know what happens when you do that. You're going to get reduced E3174 levels that act metabolate, and that's going to lead to reduced low certain mediated activity.
