If you haven't used the ford / doctor command inside of Claude code, you are probably falling behind because the way you should have Claude code set up has changed drastically over the last few weeks with these new models.
Now, this doctor command will do four big things for you, and it's going to get Claude code set up in a way to best use things like Opus 5 and Fable 5.
Now, the first thing it's going to do is check your setup health. Do you have random, duplicated, or broken files inside of your Claude system that are just slowing you down? It's going to get rid of those.
Number two, and arguably most importantly, it's going to trim your plugins. This includes skills and automations. It's going to take a look at all of your logs over the last 30 plus days and see what skills you're actually using. If you have skills that have been sitting there for months and haven't been used at all, guess what? It can get rid of them, and it's going to free up your context window because of that, which again gives you better outputs down the line.
Step three, it's going to fix your claude.md. Gone are the days where you need some super dense claude.md for all your projects specifying exactly how Claude needs to work. Less is more these days when it comes to these better models, so chances are if you haven't updated yours, it's extremely bloated.
Now, last but not least is pre-approving commands. If you don't run it and bypass permissions, chances are every once in a while Claude will ask your permission to do certain things. If it's asking you for permission for the same thing over and over again, the doctor command will check that and actually give it access all the time so you're not constantly prompted to tell Claude, "Hey, you're allowed to do that."
