This ATC Instruction Was Misunderstood — With Fatal Consequences

Sukarno Hata International Airport in Tangarang, Indonesia buzzes with activity on a Friday at noon. At 12:40 local time, 222 passengers and 12 crew members board a Garuda Indonesia Airbus A300 bound for Madan, North Somatra. It's a routine 2-hour domestic flight. Families, diplomats, and tourists step onto the plane as ground staff load luggage under the bright sun. In the cockpit, the pilots prepare for what seems like a simple flight, unaware that the northern skies hold a fatal surprise.

September 26th, 1997. Sukarnoada International Airport, Indonesia. It's just past noon and Garuda Indonesia flight 152 is beginning to welcome its first passengers. Sukarno Hada Airport, located 20 km west of Jakarta, is operating under clear skies with a temperature of 30° C and high humidity typical of the Indonesian tropics. This hub, named after presidents Sukarno and Hada, is the busiest in Indonesia with two parallel runways and a bustling terminal. The flight, a common route in Indonesia, begins by flying over the Java Sea before crossing Sumatra from south to north.

The aircraft is an Airbus A300, registration PKG AAI, delivered to Garuda, Indonesia in November 1982. This twin-engine jet, 54 meters in length, is powered by two Pratt and Whitney JT9D59A engines, each producing 50,000 lbs of thrust, and has a capacity of 250 passengers in a two-class configuration, with 27,851 flight hours and 16,510 cycles. Its last inspection was in August 1997 with no reported issues. It's an FFCC model, a modified version of the A300 that eliminates the flight engineer station, simplifying and relocating the relevant controls to the overhead panel between the two pilots.

Garuda Indonesia, founded in 1949 as the national airline, operated 60 aircraft in 1997. Airbus A300s for medium-haul routes, Boeing 747-200s for long-haul, DC-10-30 and Boeing 737-300s and -400s for regional service, and Fokker F28s and DC9s for short-haul flights. Today, Garuda remains the flag carrier, based at Sukarno, and a member of SkyTeam. After expanding in the 1990s, the Asian financial crisis and several accidents shrank its global network. It currently operates 74 aircraft: 39 Boeing 737-800s, 10 Airbus A330-200s, -300s, -900s, Neos, six Boeing 777-300ERs, and several ATR72-600s for smaller routes.

In the cockpit, Captain Hans Ratchmo, 42, is in command, with Garuda since 1978. He has 11,950 flight hours, 4,500 of them on the A300. The first officer, Tata Zoolaldi, also 42, joined the airline in 1985 after working as a flight engineer. He has a total of 2,500 flight hours with 1,000 on this model. The crew includes 10 flight attendants, and together with the 222 passengers, there are 234 people on board flight 152. Most passengers are Indonesian (180), along with Malaysians, Japanese, and Europeans, including diplomats on route to Madan, a growing industrial city.

At 12:40, the Garuda A300 began accelerating down the runway. It lifted off smoothly and started climbing toward its cruising altitude of 35,000 ft for the short flight of just over 2 hours. The route crosses the Java Sea at 460 knots (850 km/h), a 1,400 km journey to Madan.

By 13:50, just over an hour after takeoff, flight 152 checks in with Madan approach and begins its descent from 25,000 ft. As they descend and fly deeper into the island, the pilots begin to notice patches of thick haze below them. In September 1997, Indonesia is facing an environmental crisis. Forest fires in Sumatra, worsened by El Nino, have been blanketing the island in toxic haze since August. On the day of flight 152, visibility in Madan drops to 400 m, well below the 800-meter minimum required for landing, according to reports from Palonia Airport. While Jakarta enjoys clear weather, in Sumatra, smoke reduces air quality to dangerous levels, impacting both flights and public health.

At 14:00 local time, Madan approach is operating under intense pressure, with controllers handling traffic in critical conditions as flight 152 confidently approaches from the south. The flight had been uneventful until the aircraft neared Madan through the thick smoke. The runway in Madan is aligned southwest to northeast. Approaching from the south, there are two possible descent paths to runway 05: from the left or from the right. The pilot's approach charts only included the left-side approach, and Captain Wyo, a seasoned pilot with many previous visits to Madan, had only ever used this route.

However, another flight was departing from Madan, cleared for takeoff on runway 23 and set to turn north afterward. Because of this, air traffic controllers couldn't allow flight 152 to approach from the left side of the airport, as it would conflict with the outbound traffic. Instead, they directed Garuda 152 to approach from the right side, an unfamiliar path for the pilots.

At 14:13, the crew is cleared for an ILS approach to runway 05 with instructions to turn left to a heading of 240° to intercept the localizer. But there was a critical error in this transmission. When the air traffic controller cleared the flight for approach, he mistakenly used the wrong call sign, saying, "Merpati 152, turn left heading 240 to intercept ILS runway 05 from the right side. Traffic rolling on runway." In fact, there had been a Merpati Nusantara Airlines flight with the same flight number, 152, in the area not long before, likely the reason for the controller's slip. Since the controller began his transmission with "Merpati" instead of "Indonesia" or "Garuda," the crew ignored it, assuming it wasn't meant for them.

Moments later, after receiving no reply, the controller realized his mistake and asked, "Indonesia 152, do you copy?" This time, flight 152 responded and asked the controller to repeat the instruction. But when he did, he left out a crucial detail: the note about the approach being from the right side. Without that information, the crew assumed they were flying the usual approach from the left side. This led Captain Wyo to remark to his first officer that the instructed left turn seemed premature based on his experience. "The turn is too early. Request confirmation that we're clear of mountainous area." "Indonesia 152, affirmative."

About 2 minutes after the left turn, the controller contacted the Garuda flight again. "Indonesia 152, turn right heading 046 to intercept the glide slope." "Roger, heading 046." But Captain Wyo, who was flying the aircraft, turned left instead of right as instructed. The captain still believed they were conducting the left-side approach. His mind was operating on what he had done dozens of times before, which required a left turn at that point in the flight. And instinctively, he did so, even though the controller had told them to turn right.

Before First Officer Zooli could notice the mistake, Captain Wyo said he was too hot and asked him to adjust the air conditioning, which diverted his attention from the instruments and prevented him from noticing the captain's error. After 30 seconds of turning left, First Officer Zooli finally checked his instruments and saw that the aircraft was turning in the wrong direction. "Turn right," he told Captain Wyo, prompting the captain to hesitate and radio the controller. "Indonesia 152, can you confirm if we should turn left or right to heading 046?" The controller repeated the instruction to turn right, and Wyo began to reverse the direction of the incorrect left turn.

But the controller was using a slow, outdated radar that only refreshed every 12 seconds. Seeing the plane still turning left on his screen, he asked 152, "Confirm you are turning left now." Confused by this message, Captain Wyogo replied, "We are, we are turning now." At that moment, the aircraft was actually turning right. The confusion between the pilots and the controller had reached historic proportions.

Meanwhile, the aircraft was descending below 2,000 ft without clearance, likely due to an error in inputting the altitude into the autopilot. Distracted by the conflicting instructions and the extremely low visibility, the pilots failed to notice that they had dropped below 2,000 ft, the minimum safe altitude for the area. Suddenly, First Officer Zualdi noticed the altitude and pointed out that they were descending too much. Captain Wyo apologized, then applied full thrust and began climbing.

Just 5 seconds after Captain Wyogo applied full power, the tip of the right wing struck trees at the top of a ridge at an altitude of 1,550 ft, shearing off 1.5 m of the wing and part of an aileron. Someone, likely First Officer Zooli, shouted, "Pull up! Pull up!" But the aircraft, now damaged and uncontrollable, quickly banked to the right and lost lift before crashing into a ravine about 500 meters beyond the ridge. The impact destroyed the plane and instantly killed all 234 people on board.

At 14:45 local time, villagers in Buan Nabar heard a loud crash and saw smoke rising through the haze. The crash site, a forested hill 30 km from Madan, was inaccessible by road. Rescue teams arrived at 16:00 after cutting through dense vegetation. The wreckage was burning among the trees, the fuselage shattered into pieces, and the bodies unrecognizable.

On September 28th, the black boxes were recovered and sent to the BEA in France. What happened? The Indonesian National Transportation Safety Committee (NTSC) launched its investigation on September 26th, 1997, with support from France's BEA and Airbus, and published the final report in December 2000. The main cause was pilot error. The captain turned left to a heading of 215 instead of right to 046 to intercept the localizer for runway 05, taking the aircraft into mountainous terrain.

At 14:30, Madan approach instructed a right turn and descent to 2,000 ft. But a misunderstanding with Madan Tower, compounded by a radar system with a 12-second delay, created confusion about the plane's position. The CVR reveals the captain saying, "Where's the localizer?" and the first officer responding, "I don't see it," just 30 seconds before impact. Despite this, they didn't correct course.

Although visibility was down to 400 m (below the minimum), the captain chose to continue the approach rather than divert. The GPWS, a basic 1982 version, failed to alert in time. It sounded just 14 seconds before impact at 1,550 ft. But the uneven terrain, 600 to 700 m high hills, didn't trigger earlier warnings. According to the BEA, a modern EGPWS would have detected the danger 30 seconds earlier. The FDR confirmed a controlled descent at 150 knots with no mechanical failure. The aircraft was in good condition following its August inspection.

The NTSC criticized the lack of CRM training. The first officer also didn't challenge the captain's decision to continue the approach despite the poor visibility. Contributing factors included the haze from forest fires, which had reduced visibility since August, and Madan's terrain with hills south of runway 05 requiring precise navigation. Madan's radar, with its delay, didn't reflect the aircraft's real-time position, and ATC assumed the plane was right of the localizer when it was actually south of it.

The crash, which killed 234 people, became a turning point for Indonesia, triggering urgent aviation safety reforms. The NTSC recommended enhanced CRM training at Garuda, which was implemented starting in 1998. Indonesia upgraded Madan's radar in 1999, reducing the delay to 5 seconds, and required EGPWS on commercial aircraft by 2002. Visibility minimums during haze were raised to 800 meters, and ICAO adopted stricter global standards following this and other 1997 crashes. Garuda revised its approach manuals, and Madan updated its procedures for runway 05, known for its challenging terrain.

Flight 152 wasn't just an accident. It was a tragedy rooted in human error. Wyo and Zoolaldi, confident in their experience, became lost in the haze, and 234 lives paid the price.
