Donc, la première question que nous avons ici : comment pouvons-nous planifier les traitements hormonaux pour les pilotes ou les hôtesses de l’air qui travaillent des horaires de nuit ? Quand devraient-ils recevoir de la mélatonine, du cortisol et de l’hormone thyroïdienne ? Eh bien, c’est très simple, vous suivez simplement l’heure normale d’ajout de ces hormones. Donc, s’il y a euh une journée plus longue, et que certaines hormones doivent être prises le matin ou la nuit, eh bien, vous prendrez les hormones le matin, et vous prendrez celles qui doivent être prises le matin, et vous prendrez les hormones qui doivent être prises le soir également au même… au même moment, mais lorsque vous arrivez probablement la nuit quelque part. Donc, euh, fondamentalement, la mélatonine est prise au coucher. Donc, si vous arrivez quelque part euh dans un fuseau horaire différent, vous prendrez cela. Le cortisol est pris au réveil et à midi. Eh bien, vous prenez du cortisol à nouveau au réveil, à ce déjeuner, lorsque vous commencez votre journée. La thyroïde également est au réveil. Vous ne changez donc pas vraiment l’heure. Cependant, si vous avez une journée plus longue, vous aurez probablement besoin d’un peu plus de cortisol. Il y aura donc un ajout de cortisol pendant la journée. Si vous avez une journée plus courte, vous devrez prendre moins de cortisol en doses, afin que vous puissiez, lorsque euh… la nuit plus courte arrive plus tôt, prendre du cortisol euh… moins, afin de ne pas rester éveillé. C’est assez facile. Je vais même vous montrer quelque chose. Je vais arrêter de partager mon écran et je vais essayer d’obtenir exactement un fichier que j’ai normalement ouvert, qui est le fichier Jet Lag. Donc, encore une fois, je vais partager mon écran pour cela. Euh… ouvrez-le. Je ne pense pas que vous le voyiez pour le moment. Donc, laissez-moi juste jeter un coup d’œil ici, et je pense que ce fichier va vous enthousiasmer. Partager. Donc, fondamentalement, que propose-je aux personnes qui risquent d’avoir un décalage horaire ? Parce que lorsque je vais quelque part, mon décalage horaire est minimisé par ce type de traitement. Je montre d’abord qu’il y a quelques mesures de style de vie à prendre. Vous devez, par exemple, mettre beaucoup plus de lumière le matin, où que vous soyez, qu’il s’agisse d’une journée plus courte ou plus longue, vous avez besoin de beaucoup plus de lumière le matin, cela règle votre rythme circadien… Quoi ? Il n’y a pas de son ? On dit… non, je n’ai aucun son. Je ne vois rien. Donc, laissez-moi juste vérifier… Non, l’audio fonctionne. Je vais continuer. Pour les personnes qui n’ont pas d’audio, je suis désolé. Ça fonctionne bien ici. Ils vont essayer de le réparer pour vous. Donc, laissez-moi… aller plus loin et partager à nouveau mon écran. Et voici. Donc, fondamentalement, la lumière est très importante. Le plus de lumière possible lorsque vous arrivez quelque part, et le matin, vous devriez bouger, sortir. Il y a beaucoup plus de lumière à l’extérieur qu’à l’intérieur pendant la journée. Et aussi très important d’éviter les sucres, l’alcool, car ils diminuent beaucoup les hormones, et spécifiquement aussi les hormones en cas d’insuffisance surrénale. Évitez également les fruits frais entiers, car euh… au moins dans la seconde partie de la journée, sinon vous devrez aller aux toilettes très souvent. Et il y a aussi de la vitamine C dedans qui vous réveille. Vous devriez dormir physiquement dans l’avion, où que vous alliez, même si la journée est courte ou longue, cela aide beaucoup à se sentir bien. Et puis vous avez aussi une série de déficiences hormonales à corriger. Il faut prendre un peu plus d’hydrocortisone si le patient, ou vous, prenez ce traitement. Si vous prenez de l’aldostérone ou de la 9-alpha-fluorohydrocortisone, vous devez également augmenter la dose le premier jour où vous êtes… les six premiers jours fondamentalement, où que vous alliez avec un fort décalage horaire. Aussi, la mélatonine doit être augmentée, et cela peut être augmenté jusqu’à cinq fois plus les trois premiers jours, puis les trois jours suivants, lorsque vous arrivez quelque part, vous devez diminuer la dose, mais prendre toujours une dose plus élevée que la normale. Aussi, au lieu de la mélatonine, soit du 5-HTP, du tryptophane, peuvent être pris à des doses de 5 g, entre 25 et 100 mg, tandis que la dose de tryptophane est d’environ 500 mg. Aussi, ce que je fais lorsqu’il s’agit d’un décalage horaire sévère, je prends du GABA, car cela me garde endormi plus longtemps. Donc, 500 à 1500 mg par jour. Et vous pouvez également réduire les envies de sucre grâce au chrome 5 HB, mais c’est un traitement mineur. Mais le traitement le plus puissant est la mélatonine, l’hydrocortisone, et tous ces traitements supplémentaires à prendre pour faire face au décalage horaire les six ou sept premiers jours, où que vous alliez, pour un décalage horaire sévère. Évitez également la caféine, le chocolat et le thé. Ils n’interfèrent pas aussi bien que les gens le pensent pour le décalage horaire. Donc, euh, ils diminuent en fait les niveaux d’hormones. Je vais fermer ça. J’arrête de partager mon écran, et je vais à nouveau partager la présentation PowerPoint. Et me voilà.

Donc, une autre question ici est : que recommanderiez-vous pour traiter le lichen plan ? C’est le lichen plan. Vous voyez, il y a une fibrose autour de la vulve, et cela même… ferme l’ouverture du vagin vers l’extérieur, et c’est très douloureux, car la fibrose survient après beaucoup d’inflammation. Et fondamentalement, ces patientes sont privées d’androgènes. Les femmes ont besoin, pour les zones génitales comme la vulve, d’avoir des androgènes. Et le traitement le plus classique qui est donné est la dihydrotestostérone à 2,5 %, la dihydrotestostérone en gel que nous avons en France, et que nous pouvons également avoir dans les pays européens depuis la France, et cela s’appelle Andractim. Cependant, je ne le conseille pas vraiment, car c’est douloureux. Un gel… vous ne mettez pas de gel sur la vulve, surtout si elle est enflammée. Vous devez donc d’abord le mettre dans votre main, en tant que femme, vous soufflez dessus, l’alcool s’évapore, et ensuite vous pouvez l’appliquer, mais certaines femmes trouvent quand même que… euh… que ça fait mal. Normalement, il faut le prendre deux fois par jour en couche très fine. Mais un meilleur traitement, je pense, est la testostérone à 1 %, à appliquer deux fois par jour, en crème. Il doit s’agir d’une crème, comme vous le voyez ici, crème, crème à 1 %, et cela donne le même type de résultat d’après mon expérience. Donc, fondamentalement, cette crème à 1 %, une crème liposomale, est parfaite. Mettez toujours une fine couche lorsque vous mettez quelque chose de local avec une hormone, surtout une hormone sexuelle, toujours une fine couche, c’est vraiment, vraiment important.

Et ensuite, que recommanderiez-vous pour traiter l’hidradénite suppurée ? Donc, qu’est-ce que c’est exactement ? C’est exactement une inflammation de la… j’avais une image ici, apparemment, elle n’est pas là, mais une inflammation, par exemple, de la zone des aisselles, et euh… ces glandes sudoripares sont infectées principalement par ce qu’on appelle le staphylocoque doré. Et l’un des meilleurs traitements, et je me suis moi-même récemment fait une infection comme ça, c’est la fusidine, la fusidine à 2 %, très efficace contre ces endroits. Vous pouvez parfois aussi la prendre par voie orale. Cependant, un antibiotique par voie orale peut prendre neuf mois avant que votre microbiome ne se restaure. C’est donc toujours une seconde option de prendre un antibiotique par voie orale. Mais s’il s’agit d’une infection chronique grave, il peut être nécessaire de l’utiliser. Cependant, deux fois par jour, localement, la fusidine, en quelques jours, élimine normalement ce type d’infection. Si c’est une personne qui a cette hidradénite suppurée chronique, et toute infection chronique comme la cystite, la grippe facilement, et des choses comme ça, je donne à ces patients… le Timogen Alpha-1. Le vaccin est vendu, par exemple, en Chine, en Italie, et de nombreux pays peuvent l’obtenir de l’Italie ou de l’un des pays d’Asie qui le possède. De nombreux pays le possèdent. Et la dose que j’appliquerais est de 0,05 mg par jour, ce qui est une prise quotidienne, est beaucoup plus efficace qu’une fois par semaine, par exemple. Et vous appliquez cela pendant plusieurs mois, lentement et progressivement, votre système immunitaire est stimulé, et la personne n’a plus d’infection, ou presque plus d’infection. C’est donc vraiment un traitement qui peut donner… si un patient a chroniquement ou de manière récurrente ce type d’infection des glandes sudoripares.

Maintenant, une autre question par le Dr Allon du Brésil, à propos de la croissance : ai-je besoin de tests de dépistage du cancer de la croissance avant de traiter des personnes ayant une faible croissance ? Par exemple, le CA 225 est pour le cancer du sein, et le CA 19-9 et d’autres marqueurs tumoraux aussi. Pas vraiment. Vous n’avez pas besoin de rechercher plus de cancer que pour d’autres traitements. Je fais un dépistage du cancer du sein… euh… pas nécessairement avec des marqueurs tumoraux, car ces marqueurs tumoraux sont souvent utilisés lorsqu’une personne a eu un cancer et que le cancer a été retiré, pour voir s’il réapparaît ou s’il est toujours en cours. Et ils ne sont pas très utiles pour le dépistage du cancer. Habituellement, s’ils sont plus élevés, ce niveau élevé disparaît après deux ans chez une personne où aucun cancer n’a été détecté. Il est donc rare que cela aide à détecter un cancer. Je fais donc principalement un dépistage physique du cancer en faisant un contrôle des seins, en demandant à la patiente, s’il s’agit d’une femme, de faire une mammographie chez son gynécologue. Mais je vérifie moi-même les seins, et je peux aller plus loin avec d’autres examens, une échographie ou même une mammographie, ce qui est assez rare, car il y a des informations selon lesquelles une femme développe un cancer là où elle a subi la mammographie, l’instrument de la mammographie qui y entre. Donc, euh… la plupart du temps, il s’agit davantage d’un examen physique ou d’un examen paramédical que de tests sanguins pour le dépistage du cancer de la croissance, mais aussi pour les hormones sexuelles et d’autres… euh… euh… traitements hormonaux que… euh… vous pourriez vouloir vérifier davantage pour le cancer. Mais l’idée est aussi que ces traitements, les traitements hormonaux, présenteraient un risque accru de cancer, ce qui n’est pas vrai en réalité. Des études ont montré que chez les personnes atteintes d’une grave carence en hormone de croissance et traitées avec de l’hormone de croissance, elles ont 50 % de cancers en moins. Les mêmes études montrent également que… euh… les patientes qui ont une carence sévère en hormone de croissance ont deux fois plus de cancer en général, tous les cancers confondus, et ont également un risque plus élevé de mourir d’un cancer, en fait quatre fois plus de risques de mourir d’un cancer en moyenne, et deux fois plus de risques de cancer que la population moyenne. Avoir une carence sévère en hormone de croissance n’est certainement pas bon, et en suivant un traitement à l’hormone de croissance, vous normalisez le risque de cancer chez ces patientes. Ne pensez pas que l’hormone de croissance entraîne plus de cancer, elle en entraîne plutôt moins.

Voici une autre question : quel est le traitement, la stratégie recommandée pour la supplémentation en testostérone chez les hommes obèses âgés ? Eh bien, les hommes obèses âgés ont beaucoup de tissu adipeux, et le tissu adipeux convertit la testostérone en œstradiol. Plus vous avez de tissu adipeux, plus vous convertissez en œstradiol. Les hommes âgés ont également tendance, même s’ils n’ont pas plus de graisse, à convertir la testostérone en plus d’œstradiol, des hormones féminines. Chez les hommes, les hormones féminines sont liées à la dysfonction érectile, aux maladies cardiaques, et aussi à l’hypertrophie de la prostate. Vous ne voulez donc pas que ces hommes aient des niveaux plus élevés d’œstradiol, vous voulez plutôt les réduire et normaliser la situation, atteindre les niveaux des jeunes adultes. Donc, des soins supplémentaires pour les niveaux élevés d’œstradiol doivent être effectués, et pour l’hypertrophie de la prostate fondamentalement. Je fais donc toujours chez les hommes un examen de la prostate s’ils ont 40 ans ou plus, et je peux… je suis capable de réduire l’hypertrophie de la prostate en mettant des suppositoires d’environ 100 à 150 mg de progestérone, et j’y ajoute parfois d’autres suppléments, mais c’est principalement la progestérone qui fait le travail de diminution locale des niveaux d’œstradiol. Et à cause de cela, euh… la prostate… rétrécit, surtout si l’homme reçoit en même temps un traitement à la testostérone, qui a également tendance à rétrécir le tissu fibreux de la prostate, tandis que les niveaux d’œstradiol ont tendance à l’augmenter. Il faut donc contrôler beaucoup plus les niveaux d’œstradiol chez ces hommes, et fournir éventuellement un antiœstrogène. L’une des raisons pour lesquelles les hommes ont également tendance à avoir plus d’œstradiol lorsqu’ils prennent de la testostérone est qu’ils boivent du café ou de l’alcool, et cela peut doubler… cela peut entraîner une augmentation d’au moins 60 % des niveaux d’œstradiol pour un verre d’alcool, etc., et deux tasses de café. Vous devez donc faire attention à ces boissons qui ne sont pas non plus bonnes pour d’autres raisons, l’alcool diminue la quantité d’hormone de croissance, le café diminue également la quantité d’hormone de croissance. Les diminutions… euh… également d’autres hormones comme la production de testostérone, etc. Il y a donc aussi des améliorations alimentaires à apporter chez ces hommes.

Quelle est votre recommandation pour traiter une infection à levures intestinales ? Une infection à levures intestinales est quelque chose qui est vraiment une corvée pour les médecins, car la levure a une protéine de liaison, une globuline de liaison aux stéroïdes, ce qui signifie qu’elle peut se lier au cortisol et à la progestérone de préférence, mais elle peut aussi se lier à la testostérone, à l’œstradiol. Elle peut donc neutraliser les hormones, et une fois qu’elle se lie aux protéines de liaison à l’intérieur de la levure, elles inactivent également ces hormones. Mais elles les inactivent d’une manière particulière, avec des hauts et des bas. Vous avez également des troubles de l’humeur avec des hauts et des bas, de l’énergie qui monte et descend lorsqu’il y a une infection à levures intestinales. Raison de plus pour traiter cela. Comment je fais ? Eh bien, fondamentalement, j’améliore d’abord le régime alimentaire, et je conseille à la patiente de ne pas prendre d’aliments qui, lorsqu’ils tachent le système digestif, qui est humide, sans air, une situation idéale pour la prolifération des levures, euh… ces aliments comme les produits laitiers, les céréales non germées, les aliments conservés, le sucre, les sucreries. Je leur demande de ne pas les prendre, ou au moins cinq jours par semaine, pour être débarrassés de ceux-ci. S’ils les prennent tous les jours, ils sont presque sûrs d’avoir des levures à long terme. Et je leur propose également de manger en fait plus de viande, de poisson et de volaille, car lorsque vous avez de la viande, elle ne devient pas pleine de levure, elle devient pleine de bactéries qui mangent la levure. Il y a donc un équilibre dans l’alimentation, et la consommation de viande fonctionne presque comme un antibiotique contre les levures dans les intestins. Donc, ça aide, c’est utile. Mais il y a un gros problème avec les gens qui mangent de la viande et du poisson, c’est qu’en fait, ils ne mâchent souvent pas assez, et il y a alors beaucoup de gros morceaux qui vont dans l’estomac, et enfin dans les intestins, qui ne sont pas bien digérés par la personne elle-même. Vous n’absorbez donc pas beaucoup d’acides aminés, mais c’est de la nourriture pour les bactéries. Vous ne les obtenez peut-être pas, la levure, mais vous obtenez des troubles intestinaux, des gaz intestinaux avec beaucoup d’azote et des ballonnements abdominaux. Veuillez donc, si vous mangez de la viande ou si vous recommandez plus de viande à vos patients, leur demander de mâcher. Et combien de fois faut-il mâcher ? 35 fois sur une seule bouchée, jusqu’à ce qu’elle devienne complètement liquide avant d’être avalée. C’est très important, c’est l’un des principaux problèmes pour lesquels les gens n’améliorent pas leurs muscles, par exemple, euh… en mangeant plus de protéines animales, car ils ne mâchent pas assez. Mâchez donc suffisamment. Et après… j’ai besoin de fluconazole, et le fluconazole, c’est du Diflucan, 50 mg par jour pendant 40 jours, pris le matin, pour vraiment débarrasser les gens de la levure. C’est plus chez les personnes qui ne vont pas si bien.

Maintenant, quelle serait la dose idéale de ce médicament thyroïdien T3 T4, prescrit pour maintenir une femme de 74 ans qui a subi récemment une thyroïdectomie totale ? Lors d’une thyroïdectomie totale, vous retirez toute la glande thyroïde. Vous avez donc besoin de plus d’hormones thyroïdiennes. Et veuillez noter que cette patiente prend des médicaments T4, 75 µg par jour. Eh bien, cette patiente a besoin de plus de thyroïde que la patiente hypothyroïdienne moyenne, en raison de la thyroïdectomie totale. Elle a en fait aussi besoin de calcitonine, mais la calcitonine n’est plus aussi disponible. Elle n’est plus disponible en Belgique. Elle est disponible un peu à petites doses en France, mais… elle sort donc du marché graduellement et lentement. J’avais l’habitude de passer de la thyroxine à la thyroïde tous les sept, voire tous les dix jours. Donc, fondamentalement, vous commencez d’abord à maintenir la dose qu’elle prend, mais ces 75 µg par jour ne sont normalement pas suffisants. Et vous la prenez, disons, pendant 20 jours. Pendant ce temps, les 10 premiers jours, elle commencera à prendre de la thyroïde desséchée, 15 mg par jour. N’oubliez pas que les comprimés de thyroïde doivent être mâchés ou laissés fondre sous la langue pour une absorption optimale. Vous n’avalez donc pas les hormones thyroïdiennes, vous devez les mâcher et les laisser fondre afin d’avoir une absorption optimale, partiellement déjà commencée sublinguale. Puis les 10 jours suivants, elle passe à 30 mg par jour de cette thyroïde desséchée. Vous avez donc une meilleure fonction thyroïdienne. Normalement, ces patientes réagiront bien, et elles sont en hypothyroïdie à 75 µg. Elles vont mieux avec un ajout de thyroïde desséchée. Et puis vous pouvez y aller tous les sept jours, car vous allez diminuer le T4 à 50 µg de T4, par exemple, de 75 à 50, puis de 30 à 45 mg de thyroïde desséchée. Vous diminuez donc la thyroxine, augmentez en même temps cette thyroïde desséchée, vous pouvez aller un peu plus vite, puis vous passez à 25 µg par jour de T4 et 60 mg par jour de… pendant sept jours de thyroïde desséchée. Puis vous arrêtez le T4 et augmentez davantage la thyroïde desséchée, car à ce moment-là, vous allez augmenter uniquement… ce T sans diminuer le T4, car il n’y en a plus. Vous y allez ensuite à nouveau tous les dix jours, vous allez un peu plus lentement avec la thyroïde desséchée, et vous augmentez de 50 mg. Une patiente normale qui a subi cette thyroïdectomie totale, si c’est une personne de taille moyenne, a besoin d’environ 120 à 150 mg par jour, presque comme dose minimale, mais une femme de 74 ans, si elle doit être traitée lorsqu’elle prend des hormones thyroïdiennes avec des hormones féminines, pourquoi ? Parce que des études ont montré que les médicaments thyroïdiens peuvent provoquer une ostéopénie et une ostéoporose uniquement chez les femmes ménopausées, et vous ne recevez pas d’hormones féminines. Si vous recevez des hormones féminines, il n’y a pas d’ostéopénie, pas de perte de densité osseuse chez les patientes prenant des hormones thyroïdiennes. Il est donc vraiment important que cela se produise en même temps que la prise d’hormones féminines.

Voici une autre question sur l’endométriose. Le Dr Pedro du Portugal a dit : « J’aimerais discuter du cas d’une patiente qui a reçu un diagnostic d’endométriose. Elle est très anxieuse et a également un type de dermite sévère sans diagnostic spécifique. Voici ses résultats. » Ses résultats, comme vous le voyez, le taux de sucre est correct, l’hormone antimüllérienne est… est… est faible, et ensuite vous avez la FSH qui est augmentée. Au-dessus de 10, cela signifie souvent une périménopause, et au-dessus de 30, c’est la ménopause. Elle est donc en périménopause. Elle n’a normalement pas de syndrome des ovaires polykystiques, car la FSH est plus élevée que la LH. Sinon, s’il y a un SOPK, ce sera l’inverse. Et l’œstradiol est de 80, ce qui est faible. Elle est faible en œstrogènes, et des taux élevés d’œstrogènes sont en fait un déclencheur de l’endométriose. Ce n’est donc plus le cas ici. Mais je ne sais pas si ce test sanguin a été effectué au bon moment. Si vous effectuez un dépistage par test sanguin chez une femme qui n’est pas encore ménopausée, il doit être effectué environ sept jours avant les menstruations. Et je ne suis pas sûr que ce soit le cas. C’est probablement le cas si son âge est… je n’ai pas son âge ici… ai-je sa date de naissance ? Non, je n’ai pas sa date de naissance. Je ne connais donc pas son âge ici. Mais… la prolactine est en fait… correcte, pas élevée. Et la progestérone n’a pas été demandée. La 17-hydroxyprogestérone n’est pas de la progestérone, c’est un précurseur des androgènes. Chez certaines femmes qui ont trop d’androgènes, ce niveau est élevé. Et ici, il est plutôt faible. Il n’est pas élevé. Le précurseur des androgènes n’est donc pas élevé, mais nous n’avons pas la progestérone normale. Cela devrait être dû au fait que dans l’endométriose, en général, les taux de progestérone sont trop faibles. Nous devrions donc avoir… les taux de testostérone, les taux de testostérone libre sont très faibles. Ils devraient être en moyenne, les niveaux sains sont autour de 4, et c’est 0,9. Elle manque donc de testostérone, et la testostérone fonctionne chez la femme comme une progestérone. Et la TSH n’est pas anormale ici à ce niveau. Si elle est inférieure à 1,3, normalement, cela pourrait aller, mais ce n’est pas suffisant comme résultats ici. Et… donc, pour l’endométriose, quels sont les traitements qui sont vraiment efficaces chez ces patientes, et vous n’avez pas besoin d’intervenir chirurgicalement ? Et c’est assez réussi, mais c’est une approche multi-hormonale. Donc, ensuite, des tests plus approfondis sont nécessaires, effectués sept jours avant les règles. Vous avez également besoin de la thyroxine, du cortisol, et… et… et d’autres résultats, et de la progestérone réelle que nous n’avons pas. Et il y a deux traitements fondamentaux qui aident l’endométriose : la progestérone, parfois supplémentée par des œstrogènes, mais à faible dose d’œstrogènes, à forte dose de progestérone. Et vous devez réduire la prédominance des œstrogènes. Comment pouvez-vous comprendre cela ? Eh bien, pourquoi l’endométriose est-elle à l’origine ? Comment cela commence-t-il ? Eh bien, fondamentalement, lorsque vous avez une carence en progestérone, ce qui se passe avec l’utérus d’une femme est qu’il se contracte anormalement dans toutes les directions. Donc, s’il y a de l’endomètre, une muqueuse à l’intérieur de l’utérus, il peut, sous la pression de ces contractions anormales dans toutes les directions, monter à travers les trompes utérines à l’intérieur de la cavité abdominale. Il est donc toujours bon d’avoir un utérus détendu qui ne se contracte pas anormalement en raison d’une carence en progestérone. Donc, lorsque vous donnez de la progestérone, vous détendez l’utérus et vous arrêtez les contractions. C’est connu pour la grossesse, par exemple, les femmes enceintes peuvent avoir des contractions utérines excessives et faire une fausse couche. Et… donc, je trouve cette triple carence. Pourquoi trouve-je cette carence, et les gynécologues ne voient-ils pas cette carence ? Parce que certains gynécologues basent leur diagnostic sur des tests de laboratoire et non sur les symptômes cliniques. Et les tests de laboratoire montrent que ces patientes sont toujours dans les limites de référence, peut-être des taux de progestérone faibles, ou des taux d’hormones thyroïdiennes faibles, ou des taux de cortisol faibles, dans les limites de référence, mais toujours dans les limites de référence. Is it possible to start hormone therapy in a 75y old man or woman who has never undergone hormone treatments before? Yes, of course. How should treatment be initiated in Old the passions? Now can thos in beta be used in the place of thos in alpha 1? Well, the reason is estrogen deficiency; they suddenly are the estrogen—the pill has blocked the ovaries; the ovaries are a little weak during a certain time, six months to two years; there's certain estrogen deficiency from the ovaries; there's, um, during two years, less fertility that has been shown after stopping the birth control. Should you start taking oral contraceptives again? No, but you should take transdermal estrone, progesterone, as we have seen; topical lotion with estradiol and anti-androgen blockers, which you can put—we have seen this direct treatment—eventually MSM and biotin or B5, inter alia, same as for postmenopausal women with hair loss in patients with the…

Here's another question: What about male patients with metastatic prostate cancer, and it is so severe that it doesn't respond anymore to anti-androgen therapy? How do you manage using hormone therapy? Well, there's older studies that show that um, the you need to add testosterone, so you give testosterone in to somebody's terminal prostate cancer because it doesn't respond to anti-androgens, and you continue the blocking treatment, the antigen deprivation, and they seem to survive two times longer. So you have data on that; it uh, so actually you make them responsive against androgen deprivation, at least to the anti-cancer effect of this treatment, by giving testosterone. There's also some controversy that androgen deprivation therapy is not useful, just as Tamoxifen in women; it may increase the risk of progression to more dangerous prostate cancer, so a more severe form in this case of metastatic prostate cancer. So, um, I would give low-dose testosterone and continued androgen deprivation therapy, so there's no controversy with other doctors, and and and you will probably have your patient surviving two times longer.

Here's the question: Why treatment do you recommend for crêpey skin? Crêpey skin is skin that looks thin, wrinkled, and similar in appearance to crêpe. Well, this is from Dr. Bruno from France. This is the skin that's definitely a mixing of hormone deficiencies; you need growth hormone and IGF-1 to reverse this; you need sex hormones to reverse it; and you see that all the folds are very sharp, so sharp wrinkles; that's plasmin deficient, so you also need plasmin. You have all this information about this sort of skin in my textbook of reversing physical aging and a lot of more practical information also in the model, which is our most cell model on reversing physical aging, and I sharply recommend it to you.

Do you prefer administering vitamins intravenously or orally, and why? So I prefer giving orally because then I have a daily dose that can continue for months. Intravenous is probably very good for short-term, acute, but you're not going to do an intravenous every day, so the patient will have moments with intravenous where they have more than enough vitamins and then moments where they are deprived. So I prefer having that the patient has a daily dose. Now I do more and more recommend to stop some vitamin treatments two days a week or one week a month because I think it's good to have fluctuations and have the body not get used to always have supplements of vitamins. So I do think that it is interesting also to do what is called a window where you do not give the nutritional treatments. I wouldn't do this for hormones, but I might do this for nutritional treatments.

Dr. Rita from the United States: What is the best way to control blood sugar levels when starting human growth hormone? Well, by starting low dose, the first six months up to the first year, the growth hormone tends to block the insulin levels and that increases the sugar levels. It doesn't do that in people who are, let's say, normal, but people who have an insulin resistance, are patients with diabetes, you have to be careful, and I start with much lower doses in these patients. I may start even with 0.5 milligrams every two days during two weeks and then 0.5 milligrams during 15 days, and or even if very month I would increase the dose, but after six months, what happens is that the growth hormone has so much increased the muscles and so much decreased the fat that in patients with type 2 diabetes they generally have lower sugar levels with the human growth hormone treatment in adults and um uh lower insulin levels. So start relatively low.

Here's Dr. Via from Ukraine, and um Dr. Vas says: I'm a dermatologist with many years of experience practicing as a doctor of hardware and injection cosmetology, so in aesthetical medicine, how is it possible to switch a bioidentical hormone replacement therapy without setting pellets? Well, you just take bioidentical hormone treatment, uh, you start from age 30 with in most patients, and uh even with female hormones if it's a woman, and you go slow doses, then you increase, and if you are a beginner, maybe just start with lower doses and slowly increase to optimal dose, but you do have all the information you have in your um in the hormone handbook, so it's really a book I recommend because also to solve situations since I understand certain cancer risk from a long period of taking synthetic hormones; she wants to take only bioidentical hormones now. You do have to be careful: Is that estradiol, if it's causes and what is called in women, estrogen dominance, there might be more breast cancer, for example. If a woman has breast pain during five years, she doubles the risk of breast cancer, and breast pain, mastalgia, is actually due to lack of estrogens to an excess of estrogens, so you do have to give um, to be careful not to have breast pain, estradiol, so relatively lower dose estrogen and higher dose progesterone if a woman has that.

And how to maintain hormone balance while living in martial law? So in Ukraine, they have martial law, there's war going on, uh, you probably don't have so much supplements; I don't know exactly how you should do in Ukraine, uh, but I do have a lot of Ukrainian patients that seem to do well with treatment, but they get the treatment outside of Ukraine, so uh you, I think that there's less restriction on the frontiers for products between Ukraine and the European Union, so you probably will probably get there some pharmacies we can give you that can work eventually with you, so you get the right… Now, the hormone balance is important, so you need to learn this; you need to learn the symptoms of estrogen underdose, lower dose estrogen, progesterone too low dose or too high dose, so that you can balance these two hormones well, and but there's also a balance between uh cortisol and DHA and things like that, so you find that all in my course and the models that I highly recommend you; it will be more efficient, and we can help you to get pharmacies that can probably send to Ukraine, I think, if you need that.

20-year-old patient with PCOS, polycystic ovary syndrome, and uh she has details: She has a BMI of 25, so it's still normal weight, no insulin resistance, but there are irregular cycles earlier, but now monthly, slightly elevated total testosterone; the cycle is a bit better, but probably a little longer than 28 days; slightly elevated total testosterone; LH:FSH ratio is 2.6; that means she has polycystic ovary syndrome; it's higher than FSH, and she has more testosterone, but not much more, and irregular cycles always point that there's a lack of LH and lack of progesterone, but mostly lack of LH. She's trying to conceive; it has been treated with clomiphene, ovral, and progesterone in the luteal phase, but they need female hormones in polycystic ovary syndrome, but not too much. She conceived once but experienced a missed abortion at five weeks. What steps to recommend? Well, first, she's almost certainly thyroid deficient; thyroid hormone we restore a better ovulation; she'll make more female hormones with this in the right balance, certainly low thyroid. She might also have low cortisol, so you have to check in 24-hour urines cortisol and serum hydroxysteroids, but also in blood cortisol, transcortin, and and and free cortisol and so on, and provide probably hydrocortisone with lower doses, like 20 milligrams, two times 10 mg hydrocortisone in the morning and at lunchtime, and DHA, possibly 10 milligrams only in this case, because usually if there's polycystic ovary syndrome you have high DHA levels, so you provide that, so you make them ovulate a bit with hormones like the same treatment for endometriosis, cortisol, and you provide some estrogens and progesterone. You only start from the 18th day of the cycle because they need to ovulate before you give progesterone in order not to disturb the ovulation, and that will help. Now I've seen that women who cannot conceive often don't eat enough animal protein, so I I had four patients um, a long time ago, couldn't conceive; I asked them, uh, even not with the treatment I give, I asked them to take 50 to 100% more protein, like 250 to 300 grams of meat, and they all were conceived, and three of the four had a full-term pregnancy. So need to continue the animal protein during the pregnancy for optimal fertility. We also have a lot of information on uh fertility, what the nutrients are that can help, etc., so we we do have information that we even can give. That's Dr. Elina FIA. Thank you for your question.

Here's Eloise from uh France: And I was on Duphaston since I had aromatase during the perimenopause, and now I have no more cycles. Okay, okay, so she's menopausal, I suppose. Should I continue with Duphaston or switch back to…? The reason to take Duphaston is to have a strong progesterone effect that is 24 hours, that is longer than the… If you go on a menopausal treatment, you take transdermal estradiol, like something like 1.5 to 2 milligrams a day uh, from the first to 25th day of the month, and you take in the evening 100 milligrams of progesterone. If, with that, they are progesterone deficient symptoms and you seem to need the 24-hour effect, then you can go back to Duphaston. Now, Duphaston is… Duphaston is safe; there's 26% less risk of breast cancer, so you can continue on Duphaston; try both of those and see where you're better. Duphaston has better psychological effects; to make you sleep better if you take it early at bedtime. Duphaston is more neutral.

Here, Dr. VDM from Ukraine: In your hormone handbook, you provide growth hormone dosing regiments and target IGF-1 levels. I see a certain contradiction here: Growth hormone dosing starts low, gradually increases, and rarely exceeds one international unit a day; one international unit is 0.33 milligrams per day. A good target IGF-1 is 350 in men, yes; yet, with one month of one international unit per day of growth hormone, I was able to reach only 192. Is it sufficient for anti-aging and fat loss benefits, or should growth hormone dosing be increased, or IGF-1 long-acting, which is a synthetic form, added to reach the target of IGF-1? With long-acting IGF-1, that is a synthetic derivative with um, an additional amino acid sequence at uh, to make it longer acting, uh, you cannot increase the IGF-1 levels of the blood test of IGF-1, so you won't see the effect in the lab test. Now, why is this just 192 increase? Well, maybe that patient has a low IGFBP-3, and then it's okay; there's a ratio between IGF-1 and IGFBP-3. Now, the ratio I give: IGF-1 of 350 in men with that are sufficiently tall and muscular, you have an IGFBP-3 then of 3,000 micrograms per milliliter; they are both are in micrograms per milliliter, so we don't have that IGFBP-3 to have a good evaluation of what is happening here with this 192. Now, um, there are other hormones that you need for progesterone to make more IGF-1; you need enough insulin; you need enough testosterone. So if you don't correct the testosterone deficiency or an insulin deficiency in this patient, or you don't give enough animal protein so that IGF-1 can be made, it will not work to, by giving growth hormone, increase the IGF-1. Now, if you uh, Dr. Vitim, take it for yourself and you're more than 50 years of age, you want to… was growth hormone increase the IGF-1 sufficiently after age… at age 50, the liver is a little bit lazy, a little bit older, and doesn't make so much IGF-1, so you need to add IGF-1 in this case. Okay, uh, so you you need to give IGF-1 here if you're above at age 50 or above; if it's below, maybe giving testosterone and possibly insulin if you're very thin in aspect, uh uh, that that is sufficient to help you.

What should be done if patients develop acne even with a normal weight while using testosterone supplements? Dr. Alexander from the United Kingdom: Well, simply, acne is caused by a diet problem and male hormones, so acne can be caused by taking testosterone, and in women, DHA, men, the… not so much, mostly just the androgens, but in women with both hormones, and there's usually also another thing: There is, in general, uh, also an intake of sweets, especially chocolate, gives a lot of acne; it inflames, and uh milk products, especially yogurt, are is the most likely to give acne, and also if you eat food cooked in fat can also give acne. Uh, so you need to decrease these foods or or avoid the bad food, and then generally the acne solves. I do help them, the patient, by giving a local anti- uh, anti-antibiotic array with zinc; zinc is also anti-acne, and you put it locally, and then you have very good effect, uh um, but it's likely that a person who still has acne with the addition of male hormones, man or woman, that they are um, actually not eating good foods; they're eating too much sweets and milk products.

Dr. Marie from France: In the case of postmenopausal women taking hormone replacement therapy without any excessive predisposition to breast cancer, is it relevant to take the treatment cyclically, meaning 15 days of estrogen alone followed by 15 days of estrogen combined with progesterone to mimic the fertile cycle? Yeah, it's a little bit less than 15 days; it's uh 10, it's like from the 11th day to the 14th day, uh, actually 10 days of estrogen alone, and and then from the 15th to and 25th day of the cycle, 11 days where you take both hormones together um, to make the fertile cycle. Normally, in premenopausal women, but it's not really necessary to do it in uh menopausal women, uh, because uh um women generally when when they're postmenopausal, they don't want to have menstruation. So if you give the same days estrogen, progesterone, you generally block endometrial proliferation and don't have any uh um endometrium that will uh be capped off at the end, and there's uh they don't have… during the five days they stop at the end of the month; they take from the first to 25th days the both hormones, one in the morning, one in the evening; they don't have menstruation, and and it's safe, and women do well with that. I think it's better. Now you could say uh some women would prefer having menstruation because in their country it is better, so that they really are younger. In that case, you can do it; it does maybe have an advantage: is that if you give um uh 25 days uh estrogen per month, you do it per month, and then from the 13th to 25th day you provide um progesterone, 13th to 25th of the month uh, you provide then a higher amount of progesterone, 150 to 200 milligrams per day of progesterone, because you're not spreading it; you're putting it in the last two weeks, and uh that, almost the last two weeks, that gives a menstruation because you will have a a period with only estrogen that gives endometrial proliferation; it gives a cleaning of the uterus, so if there are cancer cells, they clean them off easier, but normally if you do a treatment where you give them both and you have some cancer cells, when you stop the treatment they disappear at the end, and it will give a little bleeding, but they're still a washing, so I think you could not do this, but it depends; you can discuss this with your patient; it might have an an advantage to have a cleaning every week, every month, of the uterus by menstruation in postmenopause.

Here's another one from Telia: Telia says: How can we manage a young teenager suffering from amenorrhea every three or four or six months? She has female hormones; has a normal hormone balance. Well, here again, I believe that this patient might be thyroid deficient, so she doesn't ovulate well, and she doesn't have a cycle, so maybe she's hyperthyroid; you need to correct the hyperthyroidism; if that is not sufficient, what I would do, and I certainly would do at this age, I would give her, not the birth control, I would give her transdermal estradiol; I will start any day, considering as the first day of the cycle or the fifth day of the cycle; I would do the fifth to 25th of the cycle, transdermal estradiol, at least one and a half milligrams, but probably three, two 2.25 milligrams, and um transdermal estradiol in the morning and during 21 days, and then I would give, during 11 days, I would add progesterone, 100 milligrams of progesterone, and she will have menstruation and will have a cycle, and that will be better, but I do believe that she probably has thyroid hormone uh deficiency, and she might need, in that, only additionally female hormones might be needed. Once hormones are given, since four months maybe a cycle will be restored. She might also be not eating enough; if you eat whole-grain bread, you have irregular cycles; it uh, so whole-grain bread has to be avoided. Um, she she possibly doesn't need enough animal protein; that lowers her fertility quite a lot, so how can you make sex hormones if you don't have any enough nutrients to make it? So also correct the balance in this patient.

What is your optimal opinion about NAD supplements? Well, I took NAD supplements; it did seem to give me some energy, and there's testing you can do, uh, but there's two forms: this, the nicotinamide adenine dinucleotide (NAD+) form, based on uh, that they provide NAD+ and not NADH, um, that is probably the best form, but the NADH, seems in chronic fatigue syndrome that you can take, seems in chronic fatigue syndrome, not to give so much efficacy and even aggravate some severely chronic fatigue syndrome, so it's not a miracle; it can be a help, but you have to see it from patient to patient, so you need to take uh um um patients, you have to try the treatment in a patient and see how it works out to be sure, but it's not the most important for fatigue; the most important for fatigue is really the hormones; it's more potent than the nutrients, except maybe iron can can give a lot of energy, but mostly hormone treatments like cortisol, like thyroid hormones, like growth hormone, are more potent than nutritional supplements, and I do almost… those I don't use much NAD, so uh, I I would say that my answer is is more from personal use that it it it it works, but but not as as good as as hormones.

Belia has another question here: A young woman of 34 years with two children, taking your birth control, Mercilon, very low-dose pill, is starting to suffer from hirsutism on her face, chest, and all over her body, four years after marriage. I think she probably has a lot of sex with her husband who has a lot of testosterone, and she's getting virilization because of that. Could be other reasons; she's maybe um developing a PCOS or so. What is the best course of action? Hormone check. Well, you need to check the female hormones, the thyroid hormones, the cortisol also because she also can have adrenogenital syndrome, that means that she doesn't make enough cortisol and then makes it compensates too much DHA and too much other adrenal hormones; we can check that also in 24 hours; you have more… the real tests to do are in my hormone handbook, so um, I would check all these hormones, and uh what she really probably needs is thyroid hormones because thyroid hormones stimulate the conversion of testosterone and male hormones to female hormones and decrease that one to dihydrotestosterone, and so I would probably give thyroid hormones; I would probably give female hormones added to the Mercilon that is too low dose that also block uh the male effects, so I can give Mercilon plus estrogen and a progesterone, so one, one milligram or 0.75 milligrams of estradiol from transdermal from the first day of the pill intake to the 21st day of the pill, and from the 11th day of the pill intake to the uh 21st day of the pill intake, I would add progesterone that also has an effect; I might give, if the problem is an excessive dihydrotestosterone and understand alconite, the metabolite of dihydrotestosterone, might give a finasteride, like two and a half milligrams, to to block that, and so there are many courses can be done. Um, I I did um in my hormone… I mentioned a bit what to do first; this and um you will see that there are a lot of things you other can do; had very great success about that, and you have also information about that in the model on female hormone therapy, where you get also all the information on cancer and things like that, how to solve the female hormone balance; I really highly recommend it because uh I cannot give all the answers here, uh there are so many questions, but you really get uh that in-depth answers.

Are you favorable to the usage of Metformin? If yes, in which situations? I'm not so in favor of Metformin; I I think it's a better drug than than if you don't take any hormones, but I think most of the effects of Metformin we can do with hormone treatment, so I'm I'm less… In any way, if it takes… decreases the glycation, the proteins they get too much sugar; if you avoid having a good diet without too much sugars, you need also less of this Metformin, so I'm I I think it's probably a good treatment, but it's not…

Un traitement physiologique euh pour moi, et je je préfère euh le faire avec des hormones plutôt qu’avec de la metformine.

Quelles sont les options de traitement actuelles pour les NPSI, les NLES ? Eh bien, je dirais que c’est un traitement presque facile, la narcolepsie, ils manquent d’hormones, en particulier d’hormones thyroïdiennes. Ils s’endorment parce que la circulation sanguine n’est pas bonne, et récemment, il y a eu plusieurs euh personnes atteintes de narcolepsie qui vont beaucoup mieux maintenant depuis qu’elles suivent le traitement, et possiblement aussi l’hormone de réveil qui est le cortisol, et et toutes ces choses, mais les hormones thyroïdiennes seules peuvent faire une grande différence, au moins si vous trouvez la bonne dose. Je dirais donc que les hormones thyroïdiennes, l’hydrocortisone s’il y a une insuffisance surrénale, sont les plus importantes.

J’aimerais des conseils sur la façon de gérer la ménopause avec des symptômes de sécheresse des yeux, du nez, de la peau, du vagin, un problème de prise de poids et un sommeil chaotique. Donc, si vous donnez des œstrogènes oraux à une femme qui a tendance à prendre du poids, vous améliorerez beaucoup de symptômes de carence en œstrogènes, mais vous augmenterez la prise de poids. Si vous donnez des œstradiols transdermiques, vous diminuerez l’augmentation de la graisse, augmenterez les muscles, vous aurez une meilleure composition corporelle, et c’est plus sûr. Donc, vous donnez à cette femme qui a tendance à prendre du poids et qui a une carence en œstrogènes, vous donnez des œstradiols transdermiques, vous n’en donnez pas une dose trop élevée à cause du problème de prise de poids, et vous donnez de la progestérone. Elles dormiront beaucoup mieux, elles dormiront aussi mieux avec des cogènes, car cela supprime les bouffées de chaleur la nuit. Donc, les œstradiols transdermiques et la progestérone amélioreront la situation.

Docteur Sham, euh j’ai plusieurs patientes que j’ai traitées avec de la progestérone pour une carence en progestérone confirmée biologiquement ou une insuffisance prémenstruelle.

Okay, je vais passer aux deux ou trois dernières questions ici, il y a eu tellement de questions cette fois. Elles ont développé des caillots sanguins pendant les règles. Euh, donc la progestérone prévient les hémorragies aux menstruations. Cet effet est guéri avec le système de défenses. Okay, alors veuillez me dire quoi faire. Eh bien, si vous avez une carence en progestérone et des règles abondantes, vous devez donner de la progestérone pendant une période plus longue du cycle. Donc, si vous utilisez de la dydrogestérone par exemple, euh la dydrogestérone dans ce cas serait meilleure, car il y a un effet de 24 heures, et lorsqu’il y a des hémorragies aux menstruations, vous devez donner un traitement plus constant. Vous commencez par exemple du 10e au 14e jour avec un demi-comprimé, 5 milligrammes ou 10 milligrammes selon la gravité ou le degré de gravité de la perte de sang et des caillots sanguins, c’est une forme plus sévère. Donc, vous pouvez commencer avec 1 milligramme du 10e au 14e jour du cycle, puis du 15e au 25e ou 26e jour du cycle, vous donnez 15 milligrammes ou peut-être 20 milligrammes de dydrogestérone, et euh avec cela, vous allez probablement résoudre l’hémorragie. Si la patiente a des règles trop rapides, elle pourrait alors, dans ce cas, avoir aussi besoin d’un peu d’œstrogènes. Vérifiez donc s’il y a une carence en œstrogènes, puis donnez une très faible dose d’œstrogènes, comme une pompe, soit 0,75 milligramme d’œstradiol transdermique. Dans un cas très grave de syndrome prémenstruel qui commence presque vers le cinquième jour du cycle, vous devrez peut-être commencer la dydrogestérone ou la progestérone dès le cinquième jour du cycle. Okay, vous pouvez faire la même chose avec la progestérone. Vous donnez euh par exemple 100 milligrammes de progestérone du 14e au, du 10e au 14e jour du cycle, et du 15e au 25e, vous pouvez donner un jour 100, un jour 200 milligrammes, ou vous pouvez donner euh 200 milligrammes par jour de progestérone micronisée. Mais en cas d’hémorragie, j’ai une préférence pour la dydrogestérone. Si vous êtes au Maroc, vous l’avez en pharmacie.

Quelles sont les options de traitement pour la gestion de l’hypogonadisme hypogonadotrophique ? J’y ai déjà répondu.

Où puis-je trouver des pharmacies pour acheter du Gronorm aussi de l’hydrocortisone au Brésil ? Il n’est pas courant d’avoir, avez-vous un contact pour l’acheter au Brésil ? Non, je ne crois pas. Je suis assez surprise que vous ne puissiez pas l’avoir, car dans presque toutes les pharmacies du monde, vous pouvez l’avoir. Demandez à votre propre pharmacie, et euh s’ils l’ont, euh il y a suffisamment d’études pour montrer que cela se fait chez les adultes, il y a euh plus de 481 études contrôlées par placebo chez les adultes avec du Gronorm, donc c’est beaucoup plus que chez les enfants. Vous pouvez toujours télécharger la liste et la montrer au pharmacien pour qu’il sache que c’est correct. Okay, il est traité pour le syndrome de déficit en hormone de croissance chez l’adulte.
