For today's video, I'll be showing you how to properly assess the skin, hair, and nails of the patient. But before we hop onto that, let me show you the equipment we'll be needing for today's assessment.
So, without further ado, let's get started.

Mom, when upon age 64.
So for today, Mom, I'll be assessing your skin, hair, and nails.
For me to be able to assess those parts, can I ask your consent?
It is completely confidential between me and the attending physician that I'll be referring you to.
Also, I'll be asking you some questions about your past history, such as your current symptoms.
Do you have any skin problems, like rashes or lesions?
How about dryness of the skin or oiliness?
Do you have bruising?
Do you have changes in lesion appearance?
How about previous problems with your skin, hair, or nails?
Any allergic reactions to food or medicine?
Okay, so what are your usual diet and exercise patterns?
What foods are you eating daily?
Oh, hi Mom, that's very good.
So, I already explained the procedure to you, Mom, and I'm handling this patient’s gown so you can change into your dress.
Please change, Mom.
Okay, so I am performing hand hygiene before I start with your assessment, ma'am.
I'm just closing the curtains, Mom, to provide privacy.
I already told the patient to change her clothes into a patient’s gown, and I already provided privacy for her.
So now, I'll be starting the assessment, but before that, I'll be wearing gloves.
I already told the patient to remove all the jewelry and any present cuticles in her fingernails.
So let's start with our assessment.
Okay, Mom, so I'll be inspecting for your color variations and your skin on both arms.
Okay, okay, okay, I'll just get the magnifying glass.
Okay, so I observe that your skin color is light brown, and I observe quite a few moles.
I also note any primary, secondary, or vascular lesions, and I observe that your skin is quite intact.
So, I'll be performing the assessment for the texture of your skin, ma'am.
Can you please hand me your arms again?
Okay, okay, Mom.
I absolutely remember that there are no present stretch marks or scars on your arms, only your normal skin.
I will assess the texture using the dorsal part of my hand.
So I'll touch your forehead, ma'am.
Okay, can you please extend your arm up high?
I have observed that your skin is warm, indicative of good circulation.

I have observed that there is no presence of leg edema, and so, Mom, I have observed that your skin is quite thick, and that's good.
For the next part of our assessment, I'll be inspecting your hair.
So, I observed, ma'am, that your nails are quite groomed and clean, with no pink tones or any loose ends.
Next, Mom, I'll just pinch this spot.

I am referring you to our dermatologist.
My advice is to take your medicines every day, like vitamins, if you have them, and eat healthy foods.
You should have a healthy and balanced diet.
Also, drink lots of water and, as much as possible, exercise.
