Metabolic health expert Dr. Ben Bickman is convinced that the main reason for belly fat isn't laziness or even poor nutrition. It's chronically elevated cortisol.
One of the most striking features of cortisol excess is the central atyposity, the accumulation of fat in the abdomen and trunk. Inside your abdominal cavity, you have visceral fat cells. What makes them different is that they contain receptors that are highly sensitive to cortisol. So when cortisol levels rise, this hormone directly interacts with those cells and starts influencing their metabolism. Cortisol activates specific enzymes that do two things at once. First, it blocks fat burning in the abdominal area. And second, it essentially pulls free fatty acids from other parts of the body and promotes their storage in the visceral region. In other words, elevated cortisol doesn't just make it harder to lose belly fat. It actively encourages your body to store more of it right where you don't want it.
Fat cells will express glucocorticoid receptors which upon cortisol binding elevates or regulates genes involved in fat metabolism. Now what's interesting is that in visceral atapost tissue so that is the fat surrounding the central organs within the abdominal space cortisol's effects are amplified at that at those central fat cells. Cortisol upregulates LPL, lipoprotein lipase. And the lipoprotein lipase is so important because that's what hydrayes or pulls off individual fats as triglycerides are circulating through on as carried on lipoproteins because if a triglyceride if which is the stored form of fat in fact stored in fat cells and as moved predominantly through blood, the big triglyceride itself isn't what moves in. And I'm acting like the three parts of the triglyceride. The triglyceride isn't what is moving from the blood into the fat cells. It has to get hydrayed or you have to pull off the individual fatty acids and that's what moves in. So there are seen there are these enzymes these which is LPL lipoprotein lipase. Well, cortisol will activate it at fat cells in the visceral space and so it's promoting that uptake. That's why it's so important to prevent cortisol from accumulating in visceral fat.
The peak cortisol surge happens within the first 30 minutes after you wake up and its effects can last for the next 2 hours. During that window, the goal isn't to suppress cortisol, but to use it up so it doesn't turn into a hormonal problem. First, focus on simple physiological tools. Don't jump out of bed the moment you wake up. Give your body about 10 minutes to fully activate its systems. While you're still lying down, spend 3 minutes doing diaphragmatic breathing. 4 seconds in, 8 seconds out. This helps calm the sympathetic nervous system and smooth out the cortisol response. Then, expose yourself to morning light. That could be a 10-minute walk outside or simply standing by an open window. Light acts as a biological signal that helps regulate your circadian rhythm and gradually slows the cortisol surge. There's also no need to rush into breakfast. Dedicate the next 90 minutes to simple household routines or focused work tasks. During this time, cortisol actually makes your brain and muscles metabolically active. In plain terms, you're in a natural fightor-flight state, alert, sharp, and ready to act. As ironic as it sounds, this can be the easiest time of day to tackle demanding work or clear through everyday responsibilities. However, it's not the best time for intense workouts, especially if you're just starting out. Heavy physical training is another stressor and can trigger an additional cortisol spike. It's also better to skip coffee during the first 2 hours after waking. Caffeine not only increases cortisol release, it also dilates peripheral blood vessels, which helps cortisol circulate more widely throughout the body.
According to Dr. Bickman, this can trigger another lesserk known effect of cortisol, increased cravings, especially for sweets and quick energy foods. Cortisol's metabolic effects do affect the brain, and there it influences appetite and food cravings. And wouldn't you know it, it particularly increases cravings for carbohydrates. Cortisol crosses the bloodb brain barrier and once again binds to gluccocorticoid receptors in the hypothalamus and the amydala. These are both regions involved in hunger and reward. So people feeling good about what they're doing. Now in the hypothalamus cortisol upregulates a protein called NPY neuropeptide Y and another one called a goody related protein AGRP and these are increased in response to the uh the gluccoid the gluccocorticoid response elements and when they're activated they stimulate appetite. At the same time, cortisol inhibits leptin signaling, reducing satiety potentially. But separate from that regulation of appetite in the amygdala, cortisol enhances dopamine release, so increasing cravings for very palatable, energy dense foods.
This specific feature of cortisol can actually be used to your advantage both in the morning and throughout the day. For example, intense hunger right after waking up can be a sign that your cortisol surge is too strong. Physiologically, you're not supposed to feel real hunger first thing in the morning. The primary role of cortisol in your body is to mobilize energy. In simple terms, it raises blood glucose levels so you have fuel available without needing to eat immediately. So, if you wake up starving, it may not be true hunger. It may be a hormonal signal telling you that cortisol is overshooting its normal range.
If we were to assign any primary function to cortisol, it would be to ensure energy availability, particularly glucose. So cortisol is a very powerful signal in promoting glycogenolysis in the liver, which is the breaking down of glycogen into glucose. When glucose rises, insulin follows. Under normal conditions, insulin also plays a role in signaling satiety. So, if you wake up feeling hungry, that hunger isn't truly physiological. It can actually be a sign of elevated cortisol. It may mean you didn't sleep well or that you're under stress. In that situation, the strategy changes. Instead of trying to use up cortisol, the goal is to calm the excess. Skip a heavy breakfast and start with a cup of green tea instead. Green tea can help smooth out the cortisol response while also taking the edge off your appetite. Skipping breakfast as part of intermittent fasting can also help regulate elevated cortisol and put it to work in a more controlled way.
Fasting creates a form of metabolic stress, but unlike emotional stress, this is a structured physiological process. During that state, your body begins searching for an alternative fuel source. And instead of relying on glucose, it shifts toward using stored fat for energy. All of these hormones that help us respond to danger, overt stressful stimuli like epinephrine, cortisol, glucagon, and others are also ones that rise during fasting or even just a low carbohydrate diet. Why? Why might that be? because the body may perceive these states as a bit of a metabolic stress. Now, that does not mean it's harmful, but it does mean that the body needs to adapt. And perhaps that's one of the best ways to look at even the term stress, what that means physiologically. It's it's a it's a stimulus that requires the body to change its approach. It needs to adapt. So, it's responding to the absence or the relative reduction in glucose by shifting to an alternative fuel source.
In essence, if you want to reduce belly fat linked to chronically elevated cortisol, you need to build a deliberate morning routine. That routine should include diaphragmatic breathing, exposure to natural light, and delaying or skipping breakfast. By doing this, you prevent cortisol from lingering at high levels and being stored in visceral fat. Instead of allowing it to accumulate and drive fat storage around the abdomen, you help your body regulate and use it properly so belly fat doesn't continue to build on its own.
