If someone has early morning awakening, they're the most likely to have depression. That signals depression.

If we take a thousand people with early morning awakening, what is the most likely diagnosis for all of them to have? Depression, no question.

If we take a thousand people with depression, what is the most likely sleep disturbance that they have? I don't know if it's actually early morning awakening. It could be hypersomnia. But hypersomnia is not specific to depression.

Negative self attitude, early morning awakening, these are two of the highly specific features of depression. Low energy, you can have in hypothyroidism. But negative self attitude is one of the things that like I think I'm a loser. Negative self attitude is like the way that you view yourself. Highly specific to depression.

Hopelessness isn't specific to depression. You can have terminal cancer and you can be hopeless. But how do you know if someone with terminal cancer has depression as well? Because their energy sucks, they have difficulty sleeping, their appetite is messed up. How do you know if they also have depression? Or if this is just cancer. Cuz hopelessness, possible in both. If they start thinking I'm a loser for having cancer, then that signals depression.
