[Music] T-cell prolymphocytic leukemia (T-PLL). Median age at diagnosis: 61 years. Median overall survival: less than 2 years.

Flow cytometry: CD2 weak, CD3 weak, CD5 weak, CD7 weak. Variables: CD4 and CD8.

Cytogenetics: 90% show TCL1, AB, and MTCP1 rearrangements. Association with ATM gene disruption.

Treatment: Watchful waiting for asymptomatic cases.

First-line therapy: Alemtuzumab, transplant in first remission.

PCP and shingles prophylaxis is required. Also, CMV PCR monitoring.
