The story begins with US Army Major Ben Marco, a man haunted by recurring nightmares. Publicly, he tells a heroic story about his time in Kuwait during the Gulf War. In his speeches, he recounts how Sergeant Raymond Shaw single-handedly saved their lost patrol from an enemy ambush, an act for which Marco himself recommended Shaw for the Congressional Medal of Honor.
But Marco's dreams tell a different, more terrifying story. Instead of a desert firefight, he sees himself and his fellow soldiers in a strange laboratory being experimented on by a mysterious scientist. In these visions, the heroic Raymond Shaw is not a savior, but a mindless puppet who on command commits terrible acts against his own men.
Marco's unease grows when he is confronted by Al Melvin, another soldier from the patrol. Melvin is disheveled and frantic, describing the exact same nightmares that plague Marco. Meanwhile, Raymond Shaw has become a popular congressman. Propelled by his politically powerful and ruthless mother, Senator Eleanor Apprentice Shaw, he is unexpectedly chosen as the vice presidential nominee at his party's convention.
Convinced that something is deeply wrong, Marco tries to investigate. His superiors in the army dismiss his concerns as Gulf War syndrome, a form of stress disorder. He attempts to speak with Raymond, who seems distant and strangely detached and denies having any unusual dreams. During his frantic search for answers, Marco discovers a small hard bump on his own back. In a desperate moment, he cuts it out and finds a tiny metallic implant, but accidentally loses it down a drain before he can analyze it.
He later finds the implant again and takes it to a scientist friend who identifies it as a highly advanced piece of technology developed by a powerful private equity firm called Manurion Global. This same firm is a major financial backer of Senator Eleanor Shaw. Marco's investigation leads him to the truth about the scientist from his dreams, a disgraced researcher named Dr. Adakus Noyle, who specialized in brainwashing and mind control for Manurion Global. Marco realizes his entire unit was captured, implanted, and brainwashed. Their memories of Shaw's heroism were fabricated and implanted in their minds, while Raymond Shaw himself was turned into a sleeper agent and assassin who can be activated by specific trigger phrases.
The conspiracy deepens when Senator Thomas Jordan, a political rival who threatens the Shaw's ambitions, is confronted with Marco's evidence. Soon after, Senator Shaw activates her son. Raymond, in a trance-like state, drowns both Senator Jordan and his daughter in a staged accident.
As the presidential election nears, Marco knows time is running out. He manages to meet with Raymond in secret. By now, Raymond's programming is starting to fail, and he is having his own flashes of the horrific truth. He understands he is a weapon that must be stopped, but he doesn't know how. During their meeting, Senator Shaw calls and uses a trigger phrase not on Raymond, but on Marco, activating him as well.
The final stage of the plan is set for the election night victory party. The plot is for Marco under mind control to assassinate the newly elected president. The bullet is meant to narrowly miss Raymond, making him appear to be the intended target. In the ensuing chaos, Marco would be eliminated and Raymond would become a sympathetic hero who would soon ascend to the presidency.
However, at the rally, Raymond fights his programming one last time. As Marco takes aim from his sniper post, Raymond deliberately steps in front of the president-elect and pulls his mother into a dance, positioning them both directly in Marco's line of fire. Understanding Raymond's final desperate act of sacrifice, Marco shifts his aim. He fires, killing both Raymond and Senator Shaw. In the chaos, federal agents who had been tracking Marco burst in and shoot him.
In the aftermath, a massive cover-up begins. To prevent a national panic, the authorities erase Major Ben Marco from history. They alter security footage to show an unknown gunman as the assassin. Marco, who survived his wounds, is secretly taken to a remote abandoned island, the very place where the brainwashing began. There, he is left to live out his days in isolation, a ghost who saved his country, but can never be known.
